腰椎间盘退变中终板骨软骨炎的低场MRI表现

摘    要:目的:探讨腰椎间盘退变中终板骨软骨炎的MRI表现及其临床意义。方法:搜集2006年10月至2007年10月间腰椎间盘病变的MRI资料。腰椎3210例,男性1310例,女性1810例,年龄14~88岁,均有不同程度的临床症状。结果:3120例腰椎间盘退变中,947例有终板炎,占29.5%,女性608人,占64%,均伴有不同程度的椎间盘退变。MRI变现分为四型。MRI表现为长T1长T2的约占60%,发生部位在腰4、5骶1椎体相对缘的占73%。结论:腰部终板骨软骨炎最易发生在腰4、5骶1椎体相对缘,其MRI的主要表现为长T1长T2,该期提示病变为活动期,通常患者的临床症状较重。MRI的表现分为四型,支持《临床放射学杂志》第151期的分型观点。MRI是检查本病的最佳方法。

Abstract:Objective: Low- field MRI findings and clinical significance of end- plate osteochondritis in disc degeneration. Method: During the period of Oct. 2006 to Oct. 2007, MRI was performed in 3210 cases with lumbar disc degeneration, of 3210 patients, 1310 males and 1810 females,aged 14 -88 years with clinical manifestations of different degree. Result: Of 3210 cases with lumbar disc degeneration , end -plate osteochondritis was identified in 947 ( 29.5% ) , of which, 608 females( 64% ). Based lesion' s signal intensity on MRI, end - plate osteochondritis was classified into 4 types. MRI findings of longer T1 and longer T2 signals was presented in 1926(60% ) , which were located at adjacement vertebral margin of lumbar 4/5, lumbar 5/sacrum 1. Conclusion: Above data,lumbar disc degeneration is often located at adjacent vertebral margin of lumbar 4/5 ,lumbar 5/sacrum 1, the main MR! findings of which shows longer T1 and longer T2 signals, it hints lesion is active, its clinical manifestations is usually heavy. MRI findings was classified into 4 types,which support the point of view of JOURNAL OF CLINICAL RADIOLOGY in 151. MRI is the best modality to detect the lesion.
