拉米夫定治疗慢性乙型肝炎临床疗效分析

目的:观察拉米夫定治疗慢性乙型肝炎的临床疗效.方法:将142例慢性乙型肝炎患者随机分为治疗组及对照组,两组均使用保肝及对症综合治疗,治疗组同时给予拉米夫定片剂,100mg/d,疗程12个月.结果:治疗12个月后,治疗组中症状复发率为2.67%(2/75),显著低于对照组的29.88%(20/67),P＜0.01.治疗组ALT复常率为86.67%(65/75),显著高于对照组的67.16%(45/67),P＜0.01.HbeAg阴转率、HBeAg/抗HBeAg阳转率分别为28%(21/75)、24%(18/75),显著高于对照组的10.45%(7/67)、5.95%(4/67),P＜0.01.治疗3个月、12个月后,治疗组HBV-DNA阴转率分别为77.33%(58/75)、85.33%(64/75),显著高于对照组的8.96%(6/67)、16.42%(11/67),P＜0.01.治疗组及对照组不良反应发生率分别为4%(3/75)、5.9%(4/67),P＞0.05,无显著性差异.结论:拉米夫定可以明显改善慢性乙型肝炎患者的症状、ALT及乙肝病毒病原学指标.

Clinical Study on the Efficacy of Lamivudine for Chronic Hepatitis B

Objective: To evaluate the clinical efficacy and safety of lamivudine for chronic hepatitis B.Method: 142 patients had chronic HBV infection were randomly divided into two groups,while being treated with liver protecting medicines and routine treatments,in which 75 patients received lamivudine 100mg once daily for twelve months. Result: After twelve months,the symptoms recurrence in treatment group was 2.67%(2/75),which was significantly lower than the control group 29.85%(20/67)(P<0.01).Patients with elevated ALT levels at baseline became normal in 86.67%(65/75) of the treatment group compared with the control group were only 67.16%(45/67)(P<0.01).The HbeAg negative conversion and HbeAg/anti HbeAg seroconversion of treatment group were respectively 28%(21/75) and 24%(18/75),which were significantly higher than the control group 10.45%(7/67) and 5.95%(4/67)(P<0.01).The negative for HBV-DNA in treatment group was 85.33%(64/75) compare with the control group 16.42%(11/67)(P<0.01).The overall incidences of adverse events of two groups were similar.Conclusion: Lamivudine is effective in improving symptoms,decreasing ALT and clearance of virus index of chronic hepatitis B.
