老年肥胖糖耐量减低患者网膜素-1的表达水平及临床意义

目的:探讨老年肥胖糖耐量减低患者网膜素-1(omentin-1)的表达水平及其与糖脂代谢、胰岛素抵抗、炎性因子等指标的相关性.方法:32例正常糖调节正常体重患者(NGR-NW)、29例正常糖调节肥胖患者(NGR-OB)和26例糖耐量减低肥胖患者(IGT-OB).采用ELISA测定空腹血清Omentin-1水平,并分析血清Omentin-1与BMI、血糖、血脂、HOMA-IR等相关代谢及炎症指标的关系.结果:与NGR-NW组相比较,NGR-OB和IGT-OB组的血清Omentin-1水平降低(P <0.05).空腹血清Omentin-1与FBG、2hPBG、HbA1C、HOMA-IR 和hs-CRP呈显著负相关(r=--0.766、-0.836、-0.701、-0.784、-0.715,P＜0.01或P＜0.05).多元逐步回归分析表明,2hPBG是血清网膜素的独立相关因素.结论:血清Omentin-1水平与糖脂代谢、胰岛素抵抗及炎症状态有关,因此应对Omentin-1及其影响因素加以早期干预,防止肥胖、胰岛素抵抗和2型糖尿病的发生发展.

Plasma Omentin-1 Level and its Clinical Significance inOld Patients with Impaired Glucose Tolerance

Objective: To investigate serum omentin-1 levels in old patients with impaired glucose tolerance(IGT) and the relationship between serum omentin-1level and glucose and lipid metabolism,insulin resistance indexs(HOMA-IR) and inflammatory cytokines.Method: 32 patients with normal glucose regulation and normal weight(NGR-NW group),29 patients with normal glucose regulation and obesity(NGR-OB group) and 26 patients with impaired glucose tolerance and obesity(IGT-OB group) participated in this study.Fasting serum omentin-1 levels were measured by enzyme-linked immunosorbent assay(ELISA).The relationship between serum omentin-1 levels,body mass index(BMI),blood glucose,lipids,HOMA-insulin resistance index(HOMA-IR) and other metabolic parameters and inflammation indexes were analyzed.Result: Compared to NGR-NW group,serum omentin-1 level decreased in NGR-OB and IGT-OB groups(P<0.05).Serum omentin-1 level was negatively correlated with FBG,2hPBG,HbA1C,HOMA-IR and hs-CRP(r=——0.766,-0.836,-0.701,-0.784,-0.715,P<0.01 or P<0.05).Multiple regression analysis showed that 2hPBG was independent related factor in influencing serum omentin-1 levels.Conclusion: Serum omentin-1 level was well correlated with metabolism of glucose lipid and inflammatory state.Therefore it is important to early intervene in the decline of omentin-1 and its influence factors which may prevent the development of obesity,insulin resistance and diabetes and provide access to treatment on obesity,insulin resistance and diabetes.
