部分性发作癫痫患者间歇期SPECT显像

目的本研究对 18例部分性发作癫痫患者进行了间歇期SPECT显像 ,探讨了该类患者脑血流灌注显像的影像特点。静脉注射99Tcm ECD30 4 0min后进行SPECT显像 ,以 2个以上的断面出现异常放射性药物分布区为阳性病灶。 14 / 18例部分性发作癫痫患者间歇期脑血流灌注显像结果为阳性 ,阳性检出率为 77.8% ,其中 13例表现为局部脑血流灌注减低。 14例阳性检出者中双侧病变者 7例 ,单侧病变者 7例 ,各占 5 0 %。 14例阳性检出者共检出病灶 33个 ,其中 18个病灶为某一叶的双侧对称性改变 ,占 5 4 .5 % ;15个病灶为单侧病变 ,占 4 5 .5 %。双侧病变者中额叶病灶数最多 ,单侧病变者中颞叶和枕叶病灶占多数。本研究结果显示 :本组患者间歇期脑血流灌注显像大多表现为局部脑血流灌注减低 ,可表现为单侧或双侧改变 ,具有多部位、多病灶特点。

SPECT In Patients With Diapauses Partial Seizure Epilepsies

In this study, SPECT perfusion imaging was performed in 18 patients with diapauses partial seizure epilepsies, and the imaging characteristic was discussed. SPECT perfusion imaging was performed after 30-40 minutes by mainlined 99Tc m-ECD. The positive imaging result was affirmed when an abnormal regions of radiopharmaceutical distribution was found on more than 2 slices. 14/18 (77.8%) patients with diapauses partial seizure epilepsies were positive in SPECT perfusion imaging. Among them, 13 patients images showed local decreased perfusion regions. Pathological changes in double hemisphere were found in 7/14 patients, and Pathological changes in unilateral hemisphere were found in 7/14 patients. In total, 33 focuses were found in 14 patients. 18/33(54.5%) focuses were bilateral and symmetry, and 15/33(45.5%) focuses were not. Most focus distributed on prefrontal lobe in patients with pathological changes in double hemisphere. However, more focus distributed on temporal and occipital lobe in patients with pathological changes in unilateral hemisphere. The results showed that local decreased perfusion regions presented in most patients with diapauses partial seizure epilepsies, and pathological changes may found in unilateral or double hemisphere, and the focuses were excessive and extensive.
