连续硬膜外腔阻滞镇痛对分娩的影响

目的探讨低剂量布比卡因加芬太尼硬膜外腔阻滞镇痛效果及对产程、母婴状况的影响。方法用 0 .12 5 %布比卡因加芬太尼 (1.5 μg/ml)硬膜外腔阻滞镇痛的 10 0例初产妇为研究组 ,未行镇痛术的 5 0例同等条件初产妇作为对照组。观察镇痛效果及用药前后产妇生命体征的变化 ;并对两组的产程时间、分娩方式及新生儿Apgar评分进行比较。结果分娩镇痛的有效率达 10 0 % ;研究组产妇用药期间的生命体征 ,包括呼吸、脉搏、血压及血氧饱和度 ,与用药前比较无显著性差异 (P >0 .0 5 ) :两组的活跃期时间、第二产程时间、器械助产率及新生儿Apgar评分均无显著性差异 (P >0 .0 5 )。结论 0 .12 5 %布比卡因加芬太尼(1.5 μg/ml)用于硬膜外腔阻滞镇痛是一种安全、有效的无痛分娩术

The Effects of Continuous Epidural Analgesia on Labor

Objective To investigate the painrelief effects of 0.125% bupiva caineplus 1.5μg/ml fentanyl for epidural analgesia during labor and its influence on the labor course and both mothers and infants. Methods 100 nullipara women were administered epidural analgesia with 0.125% bupivacaine and 1.5μg/ml fentanyl during labor(study group). Another 50 nullipara women were not administered any labor analgesia(control group). The degree of labor pains and the change of vital signs during labor of the women in study group were observed. The labor course, delivery ways and Apgar scores were compared between the two groups. Results The rate of painrelief reached 100%. The vital signs of pregnant women in study group remain consistant after epidural analgesia. There were no significant difference in the duration of active phase the second stage of labor,in the Apgar scores and the delivery ways between the two groups, Conclusion The analgesia delivery of epidural analgesia with 0.125% bupivacaine and 1.5μg/ml fentanyl is safe and effective,which does not decelerate the progress of the active phase and the second stage of labor.
