DR、CT、MRI在原发性良、恶性骨肿瘤中的诊断价值比较

摘    要:目的:探讨DR、CT、MRI三种影像学检查方法在原发性良、恶性骨肿瘤中的诊断价值。材料与方法:按照纳入标准,回顾性收集2012年10月至2013年10月均行X线、CT、MRI检查的骨肿瘤患者33例,每例患者均行手术和病理检查。两名放射科诊断医师分别对DR、CT、MRI诊断原发性骨肿瘤的价值进行评价,骨肿瘤分为良、恶性两组,与病理结果对照,统计其诊断符合率,采用统计学方法比较三种不同检查方法在良、恶性肿瘤的诊断价值。结果:33例骨肿瘤中有14例为恶性骨肿瘤,19例为良性骨肿瘤,DR、CT、MRI三种影像检查方法对原发性良性骨肿瘤的诊断价值有统计学意义(P<0.05),其中MRI和CT在良性骨肿瘤的诊断符合率均高于DR(16/19 vs 8/19,15/19 vs 8/19,P<0.05),MRI和CT的诊断符合率没有统计学意义(16/19 vs 15/19,P>0.05);DR、CT、MRI三种影像检查方法对原发性恶性骨肿瘤的诊断价值没有统计学意义(P>0.05),其诊断符合率分别为50%(7/14),57.1%(8/14),78.6%(11/14)。结论:DR、CT、MRI三种影像学检查方法在诊断原发性良、恶性骨肿瘤中的价值有差别,MRI和CT检查对良性骨肿瘤的诊断价值优于DR。

Abstract:Objective: To investigate the diagnosis value of primary benign and malignant bone tumor by DR,CT,MRI. Materials and Methods: According to the inclusion criteria,Thirty-three patients with bone tumors were underwent DR,CT and MRI examination simultaneously from October 2012 to October 2013,which were proved by operation and pathology. Based on three types of images,two radiologists evaluate the DR,CT,MRI in diagnosing primary bone tumors according to inclusion and evaluation criteria. Compared with pathological results,they were divided into benign and malignant bone tumors. Statistical analyses were performed to determine the diagnosis value of three examinations. Results: There are 14 cases malignant bone tumors,benignant with19 cases among the 33 cases.There are statistical significance between DR,CT and MRI in primary benign bone tumor( P 〈 0. 05). The MRI and CT in the diagnosis of benign bone tumor coincidence rate are higher than DR( 16 /19 vs 8 /19,15 /19 vs 8 /19,P 〈 0. 05). There is no statistical significance for diagnostic accordance rate of MRI and CT( 16 /19 vs 15 /19,P 〉 0. 05). There are no statistical significance between DR,CT and MRI in malignant bone tumor( 50%,57. 1%,78. 6%,respectively,P 〉 0. 05). Conclusion: There are differences in the diagnosis value of primary benign and malignant bone tumors by the three examinations. MRI and CT in the diagnosis value of benign bone tumors are superior to the DR.
