彩色多普勒超声对门静脉海绵样变性的病因学诊断价值

目的 探讨彩色多普勒超声对门静脉海绵样变性的病因学诊断价值，以及其声像图改变。方法 应用彩色多普勒超声检查63例门静脉海绵样变性患者，观察门静脉系统的血流状况，结合二维超声，对门静脉海绵样变性的病因作出诊断。结果 彩色多普勒超声显示门静脉主干及周围分支呈蜂窝状、多条迂曲扩张的管状结构，内充满门静脉样血流。引起门静脉海绵样变性的原因主要包括肝硬化伴门静脉血栓和门静脉高压脾切除术后，另外还有肝癌伴门静脉癌栓、门静脉炎和原因不明。结论 彩色多普勒超声对门静脉海绵样变性的病因学有较高的诊断价值。

Etiologic diagnostic value of color Doppler ultrasonography on cavernous transformation of the portal vein

Objective To evaluate the diagnostic value by color Doppler ultrasonography in determining the cause of cavernous transformation of the portal vein(CTPV).Methods 63 cases with CTPV were examined by color Doppler ultrasonography.The blood flow of portal vein,morphology and flow character of affected veins were observed.The cause of CTPV of each case was determined.Results color Doppler ultrasound showed honeycomb-shaped,the multi-strip expansion tube structure within filled with the portal vein type blood stream around main portal pain and branches.The causes of CTPV mainly included liver cirrhosis with portal vein thrombus and the portal vein high pressure after splenectomy,moreover liver cancer with portal vein cancer-thrombus,the pylephlebitis and the reason is unclear.Conclusion Color Doppler ultrasonography have a significant value in determining the cause of CTPV.
