经单鼻孔蝶窦入路显微手术治疗垂体腺瘤67例

目的探讨经单鼻孔蝶窦入路显微手术垂体腺瘤切除的方法和效果。方法总结2008年8月至2010年8月67例经单鼻孔蝶窦入路垂体腺瘤手术患者的临床资料，其中微腺瘤16例，大腺瘤43例，巨大腺瘤8例。结果肿瘤全切除50例，次全切除9例，大部分切除6例，部分切除2例；其中泌乳素（PRL）腺瘤35例，生长激素（GH）腺瘤8例，混合性腺瘤PRL＋GH或PRL＋促肾上腺皮质激素（ACTH）]16例，无功能性腺瘤8例。结论经单鼻孔蝶窦入路垂体腺瘤切除术是一种微创、安全、有效的外科技术。

Transsphenoidal approach operation for pituitary adenoma in 67 cases

Objective To study the methods and evaluate the clinical effects of single -nostril transsphenoidal pituitary adenoma microsurgery. Methods Clinical datas of 67 cases of pituitary adenoma treated by single - nostril transsphenoidal approach microsurgery from August 2008 to August 2010 were analyzed retrospectively. In which, 16 cases of micro - adenoma, 43 cases of large adenoma and 8 cases of huge adenoma. Results Total resection was achieved in 50 cases,subtotal resection in 9 cases,most partial resection in 6 cases and partial resection in 2 cases, no one died. Of the 67 cases, 16 cases were PRL adenoma, 8 cases were GH adenoma, mixed type （incluing PRL plus GH type or PRL plus ACTH type） 16 cases and no functional adenoma 8 cases. Conclusions Single - nostril transsphenoidal approach microsurgery is a minimally invasive, safe and effective method for the treatment of pituitary adenomas.
