64例早发型重度子痫前期的临床分析

目的探讨早发型重度子痫前期对围生期母婴预后的影响。方法对本院64例早发型重度子痫前期患者进行回顾性分析。根据其发病孕周分成三组,即A组(孕周<28周)、B组(28周≤孕周<32周)、C组(32周≤孕周<34周)。结果早发型重度子痫前期三组患者并发症发生率随发病孕周延长而下降,但三组间差异无统计学意义;三组新生儿窒息率和围生儿死亡率均随发病孕周延长而下降,且三组间差异均有统计学意义(P<0.05);B组期待治疗时间均明显长于其他两组(P<0.05),各组患者的分娩方式均以剖宫产为主。结论早发型重度子痫前期严重影响母婴预后,在期待治疗过程中,应严密监护母胎情况,适时终止妊娠,终止妊娠的方式首选剖宫产。

Early onset severe preeclampsia of 64 cases

Objective To study the effect of early onset severe preeclampsia on the maternal and neonatal outcome.Methods A retrospective study was carried out on 64 patients with early onset severe preeclampsia.They were divided into three groups according to pathogenic week of gestation: group A,group B and group C. Results The rates of pregnant complications in early onset severe preeclampsia declined following along with the postponement of pathogenic week of gestation,but there was no significant difference between the arbitrary two groups.The neonatal asphyxia rate and perinatal infant mortality rate of three groups declined following along with the postponement of pathogenic week of gestation,and there were significant differences in statistics between the arbitrary two groups(P<0.05).Expectant treatment time of group B was significantly longer than that of the other groups(P<0.05),and cesarean section was a main method of pregnancy termination for all groups.Conclusion The early onset severe preeclampsia badly influences the maternal and neonatal outcome.During the expectant treatment,the situation of maternity and fetus should be closely monitored and the pregnancy termination be in good timing,and cesarean section is the first choice.
