| 摘 要： | 目的 探讨老年 2型糖尿病 (DM)胰岛素敏感性与心血管病致病危险因素间的关系。方法 以 114例老年 (>6 0岁 ) 2型糖尿病患者为研究对象 ,测量其身高、体重、血压、糖化血红蛋白 (HbAlc)、空腹血糖和胰岛素(IS)以及血脂 ,并行颈动脉彩超检查 ,测定颈内 (ICA)及颈总动脉 (CCA)内中膜值 (IMT)。以HOMA -IR法计算胰岛素抵抗度 (IR) ,设定IR >2 7为胰岛素抵抗组 ,<2 7为胰岛素敏感组。结果 两组患者的体重指数 (BMI)、TG、HDL -C、FIB、PAI-I间差别有显著性意义 (P <0 0 5 ) ;两组患者ICA、CCA的IMT间差别无显著性意义 (P>0 0 5 ) ;IR与BMI、HbAlc (P <0 0 0 1)、TG (P <0 0 1)、空腹血糖 (P <0 0 0 1)呈正相关 ,与HDL -C呈负相关 (P <0 0 5 ) ,与BMI、TG两项呈独立相关 (P <0 0 5 )。结论 老年 2型糖尿病患者胰岛素抵抗表现为肥胖、高血糖、高甘油三酯 ,并与凝血因素有一定关系

The relationship between insulin resistance and cardiovascular risk factors in the elderly type 2 diabetics |

| Abstract: | Objective To find out the relationship between insulin resistance and cardiovascular risk factors in elderly subjects with type 2 diabetes.Methods A total of 114 type 2 diabetic patients were studied.Levels of HbA1c,fasting plasma glucose,serum lipids,serum insulin and BMI were measured.Insulin resistance was estimated by the homeostasis model assessment(HOMA-IR).The subjects were divided into two groups according to the value of HOMA-IR.Values>2 7 were indicative of the insulin resistant state,and values <2 7 were indicative of the insulin sensitive state.Results IR was correlated with TG,BMI and HbA1c,and inversely correlated with HDL-TC.Conclusion These results indicate that elderly type 2 diabetic patients with insulin resistance are characterized by high triglyceride and BMI levels,and low HDL cholesterol levels. |
