原发性高血压患者肥胖和转化生长因子β_1的关系研究

摘    要:目的评估肥胖对肾功能的影响以及原发性高血压患者转化生长因子(TGF-β1)水平和肥胖之间的潜在关系。方法研究对象包括70例新诊断的高血压患者(高血压组)和30名正常对照者(对照组)。检测两组受检者的血糖、血肌酐、血尿酸、血脂和TGF-β1,并且收集24 h尿样,测定24 h尿微量清蛋白,计算肌酐清除率(CCr)。结果高血压组患者TGF-β1水平与对照组比较差异有统计学意义〔(563.7±72.3)pg/ml vs.(451.3±71.6)pg/ml,P〈0.01〕,而高血压组患者的CCr与对照组比较显著降低,差异有统计学意义〔(99.1±26.3)ml.min^-1.1.73 m^-2vs.(115.2±23.1)ml.min^-1.1.73 m^-2,P〈0.01〕。高血压患者中肥胖组与非肥胖组的血清TGF-β1水平〔(593.7±67.9)pg/ml vs.(533.9±68.3)pg/ml〕和CCr〔(113.2±30.3)ml.min^-1.1.73 m^-2vs.(92.9±27.5)ml.min^-1.1.73 m^-2〕以及尿微量清蛋白〔(56.8±61.8)mg/24 hvs.(15.5±11.6)mg/24 h〕间差异均有统计学意义(P〈0.05)。高血压患者的TGF-β1水平与体质指数呈正相关(r=0.359,P〈0.05),与CCr亦呈正相关(r=0.278,P〈0.05)。结论原发性高血压肥胖患者的CCr、UAE和血尿酸水平高于非肥胖患者;另外,原发性高血压患者的TGF-β1水平与体质指数、CCr呈正相关。

Abstract:Objective To evaluate the effect of obesity on renal functions and the possible relationship between transforming growth factorβ_1 (TGF-β_1) and obesity in hypertensive patients.Methods Seventy newly diagnosed hypertensive patients and 30 normal people as controls were enrolled.The levels of glucose,creatinine,uric acid,lipids,and TGF-β_1,were measured for the subject in both group.A 24-hour urine sample was also obtained so as to calculate creatinine clearance rate (CCr) and urinary albumin excretion(UEA).Results TGF-β_1 level was significant higher in the hypertensive patients than in controls(563.7±72.3)pg/ml vs (451.3±71.6) pg/ml](P<0.01),but CCr level was significant lower(99.1 ± 26.3) ml·min~(-1)·1.73 m~(-2) vs (115.2±23.1)ml·min~(-1)·1.73 m~(-2)](P<0.01).The levels of TGF-β_1(593.7±67.9) pg/ml vs (533.9 ±68.3)pg/ml,P<0.01],CCr(113.2±30.3)ml·min~(-1)·1.73 m~(-2) vs (92.9±27.5)ml·min~(-1)·1.73 m~(-2),P<0.01,and UAE(56.8±61.8)mg/24 h vs (15.5 ±11.6)mg/24 h,P<0.05]were significantly higher in the patients with hypertension and obesity than in the patients with hypertension with no obesity.The TGF-β_1 level was correlated with body mass index(r=0.359,P<0.05),and CCr(r=0.278,P<0.05) in the hypertension patients.Conclusion The increased body mass index may be associated with increased levels of CCr,UAE,and TGF-β_1 in essential hypertension patients.In addition,TGF-β_1 is positively correlated with body mass index and CCr level in patients with essential hypertension.
