中山市社区中老年人个体化健康管理模式的研究

本文以2005年8月-2007年8月到我院健康管理中心体检的所有中老人为研究对象，通过健康管理中心这个平台，利用综合手段，如健康教育宣传、电话咨询、电视和电台健康节目互动，互联网、健康评估、心理治疗．社会生活方式指导等，对中老年人群进行个体化的健康管理，以探索一条对中老年人进行个体化健康管理的模式，以便更早、更全面地控制慢性疾病的发生和发展，达到减少管理对象罹患慢性病的危险性，从而减少医疗费用的目的。

Study of Individual Health Management Mode for Middle-Old-Aged People in the Communities of Zhongshan

All the middle - old - aged people who got check - up in health management center of our hospital from August 2005 to August 2007 were chosen as the subjects. The individual health management of the middle and old aged peoples was performed in the health management center by using synthetic means, such as health education, telephone consultation, television, broadcasting health program, internet, health estimation, psychotherapy and social life style guide. It is thus to approach an individual health management mode for middle - old - aged people, in order to control the onset and development of chronic diseases, reduce the incidence of chronic diseases for managed subjects, and gain the purpose of lowering the medical expense.
