旋转数字减影血管造影术诊断颅内动脉瘤的优势分析

摘    要:目的 探讨旋转数字减影血管造影术(旋转DSA)在诊断颅内动脉瘤中的价值.方法 采用菲利浦INTEGRIS CV数字减影血管造影机对217例蛛网膜下腔出血可疑颅内动脉瘤患者行常规DSA和旋转DSA,比较二者在显示动脉瘤位置、大小、形态、瘤颈的宽窄及其与载瘤动脉的空间关系,评价旋转DSA的诊断优势.结果 本组共检查217例患者,常规DSA检出139例患者153个动脉瘤,检出率为64.1%,旋转DSA检出156例患者177个动脉瘤,检出率为71.9%,两组患者的检出率间差异有统计学意义(P<0.01);常规DSA检查95个动脉瘤清晰显示;旋转DSA检查157个动脉瘤清晰显示,排除5例常规DSA呈假阳性的患者;旋转DSA较常规DSA在显示动脉瘤瘤颈和瘤体形态及与载瘤动脉关系方面差异均有统计学意义(P<0.01).结论 旋转DSA均能在适当的角度,使动脉瘤瘤体、瘤颈形态及载瘤动脉与周围动脉关系清晰地显示出来,从而提高诊断动脉瘤的准确率.

Abstract:Objective To investigate the clinical value of rotary digital subtraction angiography(DSA) in the diagnosis of intracranial aneurysms(ICA).Methods A total of 217 patients with subarachnoid hemorrhage(SAH),suspected to having ICAs,were chosen to undergo conventional DSA or rotary DSA with Philip INTEGRIS CV-DSA.Their spatial relationships of revealing aneurysm locations,sizes,shapes and pedicle widths to the parent arteries was compared to evaluate the advantages of rotary DSA in ICA diagnosis.Results Conventional DSA revealed 153 aneurysms in 139 patients(64.1%),95 of which were displayed clearly;rotary DSA did 177 in 156 patients(71.9%),157 of which were clear,and the detection rate increased by 7.8% as compared with conventional DSA(P<0.01),and 5 positive aneurysm patients by conventional DSA were found out to be false.Rotational DSA did much better than conventional DSA in revealing aneurysm pedicles,shapes and spatial relations with parent arteries.There was significant difference between them(P<0.01).Conclusion Rotary DSA can reveal clearly aneurysm tumors,pedicle shapes and the relationship of parent arteries to peripheral arteries to improve the accuracy in diagnosis of aneurysms.
