EB病毒感染与婴儿肝炎综合征的临床研究

目的探讨EB病毒(EBV)感染在婴儿肝炎综合征(IHS)发病中的作用。方法选取45例HIS患儿为研究组,40例健康婴儿为对照组。采用ELISA法检测研究组和对照组血清EBV衣壳抗原(VCA)IgM抗体,采用PCR法检测研究组和对照组外周血单核细胞及咽分泌物中的EBV DNA,并进行比较分析。结果研究组45例患儿中有7例血清VCA-IgM抗体阳性,阳性率为15·6%;对照组40例婴儿血清VCA-IgM抗体均为阴性,两者间差异有统计学意义(P<0·05);研究组与对照组的外周血单核细胞EBV DNA的阳性率间差异有统计学意义(P<0·05),咽分泌物EBV DNA的阳性率间差异亦有统计学意义(P<0·01)。结论EBV感染是IHS的重要病因,由EBV感染导致的婴儿肝脏损害应引起临床高度重视。

Clinical Study on Epstein Barrvirus Infection and Infantile Hepatitis Syndrome

Objective To explore the role of Epstein Barrvirus(EBV) infection in the pathogenesis of infantile hepatitis syndrome(IHS).Methods We chose 45 IHS infants as the study group and 40 healthy infants as the control group.EBV DNA in peripheral blood mononuclear cell and pharyngeal secretion was detected by PCR-Southern and EBV VCA-IgM by ELISA.Results The positive rate of EBV DNA in peripheral blood mononuclear cells was 20.0%(9/45) in the study group and 35.5%(1/40) in the control group;the positive rate of EBV DNA in pharyngeal secretion was 35.5%(16/45) in the study group and 2.5%(1/40) in the control group,and there was significant difference between the two groups(P<0.01).The positive rate of EBV VCA-IgM in serum was 15.5%(7/45) in the study group and 0 in the control group,difference between the two groups was significant(P<0.05).Conclusion EBV infection is a causative agent of IHS.Infantile hepatitis syndrome due to EBV infection should be given adequate attention to.
