血必净注射液对严重创伤致急性肺损伤患者凝血功能的影响

目的探讨血必净注射液对严重创伤致急性肺损伤患者凝血功能的影响。方法选择我院收治的严重创伤致急性肺损伤患者38例,随机分为治疗组和对照组各19例,两组患者均予积极综合治疗(包括处理原发病、输血补液维持循环系统稳定、保护性机械通气维持呼吸及纠正低氧血症、对症止血和镇痛镇静、预防和处理消化道出血、抗炎抗感染、维持内环境稳定等),治疗组在此基础上于入院当天加用血必净注射液50ml加0·9%氯化钠溶液100ml静脉滴注,60min/次,2次/d,连用10d。分别于治疗前、治疗后5和10d观察两组患者血小板计数(PLT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)。结果两组患者治疗后5和10d时PT、APTT、PLT间差异均有统计学意义(P<0·05)。结论早期应用血必净注射液对严重创伤致急性肺损伤患者凝血功能有良性调节作用。

Effects of Xueb-jing Injection on Coagulation Function in Patients with Acute Lung Injury

Objective To investigate the Xuebijing injection on coagulation function in serious trauma patients with acute lung injury.Methods Choosed 38 serious trauma patients with acute lung injury in our hospital,38 cases were randomly divided into treatment and control groups each 19 cases,the two groups patients obtained comprehensive treatment(including the treating of the original disease,blood transfusion and rehydration maintain circulatory system stability,protecting mechanical ventilation to maintain correct breathing and hypoxemia,hemostasis,analgesia and sedation,prevention and treatment of gastrointestinal bleeding,anti-inflammatory and anti-infection,maintaining stability in the environment,etc).Based on this treatment group treated with Xuebijing was injected on admission day,Xuebijing injection 50 ml in 0.9% NaCl solution 100 ml intravenous infusion for 60 min one time,2 times one day,upto 10 d.Patients were observed with platelet count(PLT),prothrombin time(PT),activated partial thromboplastin time(APTT),prothrombin time(TT),fibrinogen(FIB) respectively before and after treatment 5,10 d.Results 5 days after treatment the PT,APTT,TT,PLT and 10 days after treatment the PT,APTT,PLT were difference between the two groups,The difference between the indexes were significantly(P<0.05).Conclusion Early using Xuebijing injection treated serious trauma patients with acute lung injury,which induced coagulation function benign regulatory role.
