鲍曼不动杆菌对碳青霉烯类耐药性及耐药基因型分析

摘    要:目的:了解南京地区鲍曼不动杆菌耐药特点及分析碳青霉烯酶、金属β-内酰胺酶基因型。方法:K-B法测定临床分离的73株鲍曼不动杆菌对亚胺培南、美洛培南的耐药性;PCR检测碳青霉烯酶(OXA)、金属β-内酰胺酶(IMP和VIM)耐药基因,并对OXA基因进行测序。结果:73株鲍曼不动杆菌对亚胺培南、美洛培南耐药率均为10.9%;8株耐药菌中3株是IMP型,2株是VIM型,4株是OXA型,其中有1株菌含有OXA及IMP两种基因型。4例OXA型标本经测序,在Genbank中进行同源性比对,均为OXA-23亚型。结论:目前南京地区鲍曼不动杆菌的耐药性与其携带OXA-23、IMP和VIM基因相关。

Abstract:Objective: To survey the resistance to carbopenems and genotype of carbapenem and metallo beta-lactamase of A.baumannii. Methods:The antibiotic susceptibility to imipenem and meropenem of 73 strains of A.baumannii was tested by K-B methods. OXA,IMP and VIM resistance genes were detected by PCR, and the positive products of OXA genes were sequenced. Results: The resistance rate to imipenem and meropenem of the 73 strains of A.baumannii was both 10.9%. of the 8 strains resistant to imipenem, 3 preseated with positive IMP gene, 2 presented with positive VIM gene, and 4 presented with positive OXA gene,one out of the 8 strains presented with two genotypes; the DNA sequence of 4 OXA gene positive products presented with OXA-23 subtype. Conclusion: The resistance rate to carbopenems of A.baumannii is fairly low in Nanjing, and the resistance in A.baumannii was related to OXA-23, IMP and VIM resistance genes.
