核素心肌灌注显像与冠状动脉造影结果的对比分析

摘    要:目的:分析放射性核素心肌灌注断层显像与冠状动脉造影结果二者不相符合的可能原因,以便尽可能减少核素显像的假阳性与假阴性结果。方法:回顾性分析两所医院1996-2001年行放射性核素心肌灌注断层显像和冠状动脉造影的52例患者,所有患者均行症状限制性运动负荷心肌灌注显像或双嘧达莫(潘生丁)药物负荷试验,负荷显像异常者于24h后再行静息心肌灌注显像。结果:核素显像与冠状动脉造影结果两者不相符的患者共5例,心肌灌注断层显像阳性而冠状动脉造影正常者4例,心肌灌注断层显像阴性而冠状动脉造影阳性者1例,结论:在冠心病诊断上,核素心肌灌注断层显像与冠状动脉造影之间有高度的一致性,当二者不一致时,综合考虑显像和造影结果可提高对冠心病的诊断水平。

Abstract:Objective: To analyze the possible causes of discordance between myocardial perfusion SPECT and coronary artery an-giography(CAD) in diagnosing coronary artery diseases, and reduce the fasle positive and negative results of the myocardial perfusion SPECT. Methods: Fifty two patients from two hospitals in recent 5 years were analyzed retrospectively, the exercise or dipyridamole stress myocardial perfusion SPECT imaging was detected in all patients, and the patients who had abnormal results of stress imaging were examined by myocardial perfusion SPECT imaging with 24 hours. Results: There was discordance of image results between myocardial perfusion SPECT and CAD in 5 patients. Four had an abnormal SPECT image while their CAD were normal. On the other hand one had a normal SPECT image but an abnormal CAD. Conclusion: There was a high concordance of image results between nuclear myocardial perfusion SPECT and coronary artery angiography in diagnosing coronary artery diseases; A comprehensive understanding of the discordant results of the two tests may further promote the accurate diagnosis in coronary artery diseases.
