彩色多普勒超声对腓动脉穿支皮瓣血管的研究

目的:应用高分辨率的彩色多普勒超声检测技术对腓动脉穿支的分布规律及血流动力学参数进行研究,为皮瓣的设计提供可靠依据。方法:应用高分辨率彩色多普勒超声对25例健康志愿者44侧腓动脉穿支血管的数目、起始内径、走形、穿深筋膜位置、血流动力学参数进行检测,并分组进行统计学分析。结果:双侧腓动脉共44支,检出穿动脉总数161支,每侧2～6支不等,平均3.6支。穿支穿深筋膜位置大多(81.3%)在腓骨小头最凸点下方9～23cm。将腓骨小头最突出点与外踝最突出点的距离等分为3段,深筋膜穿出点位于上段者共36支,占总数22.3%,中段者共104支,占总数64.6%,下段者共21支,占总数13.1%。穿支动脉起始处内径平均为0.95±0.23mm,收缩期峰值流速(PSV)平均为0.17±0.08m/s。最粗大穿支动脉血流峰值流速(PSV)左右侧组间差异无统计学意义(P>0.05),男性稍高于女性(P<0.05)。结论:彩色多普勒超声可直观地显示腓动脉穿支的走行分布、评价血管质量,并能在体表准确定位,可以为皮瓣的设计提供可靠的依据。

Detection on distribution and flowing dynamics of the cutaneous perforators of peroneal artery by color Doppler flow imaging
