胃癌患者外周血和骨髓血中CD44v6基因表达与胃癌微转移的研究

摘    要:目的:探讨胃癌患者外周血和骨髓血中微转移的检测和意义。方法:以CD44v6基因作指标,以半定量RT鄄PCR方法检测46例胃癌患者和6例非恶性病对照者。结果:6例对照者外周血和骨髓血中都未检出CD44v6mRNA表达,46例胃癌患者外周血、骨髓血中CD44v6mRNA的阳性表达率为84.8%和86.9%(39/46例,40/46例),骨髓血阳性表达率稍高于外周血(P>0.05),外周血中弥漫型癌血中32例有30例有CD44v6mRNA表达,表达率93.75%,而肠型癌血中14例仅有9例有表达,表达率64.28%,两者比较差异有显著性;而骨髓血中弥漫型癌32例中31例有CD44v6mRNA表达,表达率96.87%,而肠型癌血中14例仅有10例有表达,表达率为71.42%,两者比较差异有显著性。结论:检测胃癌患者外周血、骨髓血中CD44v6基因的微转移表达可提高胃癌患者临床分期的准确性,并判断预后。

Abstract:Objective: To detect the presence of micrometastasis in peripheral blood and bone marrow in patients with gastriccarcinoma and deduce its clinical significance. Methods: Preoperative peripheral blood and bone marrow specimens of 46 patients with gastric cancer and 6 controls were studied by semi-quantitative RT-PCR amplification of CD44v6mRNA. Results: CD44v6mRNA was not expressed in peripheral blood and bone marrow of controls individuals, and CD44v6mRNA expression was positive in peripheral blood in 39(84.8%) patients with gastric cancer and in bone marrow in 40(86.9%) patients. In peripheral, CD44v6mRNA expression was positive in diffuse type in 30(93.75%) patients from 32; In intestinal type in 9(64.28%) patients from 14. On the other hand, in bone marrow, CD44v6mRNA expression was positive in diffuse type in 31(96.87%) patients from32; In intestinal type in 10(71.42%) patients from 14. Conclusion: Semi-quantitative-RT-PCR amplification for CD44v6 mRNA is a sensitive and specific method for detection of micro-metastasis in peripheral blood and bone marrow, which might be of value in predicting the prognosis of the case.
