摘    要:目的 :探讨 3D- CTA成像技术在颅内动脉瘤中的应用价值。方法 :对临床怀疑颅内动脉瘤的 10 3例患者成功地进行 CT检查 ,用 3D- CTA的 5种成像技术分别进行成像 ,并与手术和 DSA做对照。结果 :95例患者共发现10 6个动脉瘤 ,3个动脉瘤 CT漏诊 ,8例 3D- CTA及 3D- DSA均为阴性。经分析统计 ,在动脉瘤的检出方面 ,以 VR技术最优 ;在动脉瘤钙化和血栓的诊断上 ,以 VE技术最优。结论 :3D- CTA的 MIP、VE及 VR技术各有其优势并可相互补充 ,从而清楚地显示动脉瘤及周围相邻空间结构并可发现血管壁及瘤颈的钙化 ,为制定手术方案提供了详细的资料 ,具有很高的临床实用价值。

Abstract:Objective To evaluate the clinical value of imaging techniques of three-dimensional computerized tomographyangiography(3D-CTA) in the diagnosis of intracranial aneurysms.Methods 103 case who suspected to be intracranial aneurysms underwent CT angiography with five imaging technique.Results wore compared with the operation and digital subtraction angiography(DSA).Results 106 Intracranial aneurysms were identified in 95 patients.3 aneurysms were not detected by CTA but identified by DSA,8 patients were negative in both 3D-CTA and 3D-DSA,Volume rendering technique(VR) was superior to other methods in the detection of intracranial aneurysms.Virtual endoscopy technique(VE)had advantage on the detection of calcification and thrombus.Conclusion 3D-CTA techniques such as VR、MIP and VE have their own predominant role and complement each other。3D-CTA provided good illustrations of aneurysm’s features including calcification of wall or neck in a aneurysm as well as its background structures,so as to provides very useful information for clinical approaches.
