| 摘 要： | 目的通过回顾病例分析，研究探讨胆总管内探查取石及ERCP取石结石引起下段胆管医源性损伤的预防和处理。方法回顾总结我院95年以来9例以及外院会诊所见26例胆总管下段损伤的病例．结合我科多年来的临床经验，对此类医源性损伤的术中预防和诊断以及早期处理提出科学严谨的理论依据。结果35例均为胆总管内取石所致损伤，33例经T管引流和十二指肠后双套管引流治愈，2例术后并发消化道大出血死亡。结论胆总管末端医源性损伤的常见原因是不正确的胆总管探查，术中及时诊断和正确处理是关键。

| Abstract: | Objective To evaluate the prevention and treatment of iatrogenic injury to the end of the common bile duct (CBD). Methods Clinical data of 35 patients with iatrogenic injury to the end of the common bile duct were analyzed retrospectively, 9 cases from our hospital and 26 cases from other hospitals. Results Iatrogenic injury to the end of the common bile duct in all 35 cases occurred in the procedure of taking out the stone, 33 cased were cured by duodenal drainage,2 patients died of digestive tract bleeding. Conclusion It is the key way that a correct determination and management was made early in the operation, and the common cause of iatrogenic injury to the end of bile duct was incorrect exploring in the common bile duct. |
