妊娠期糖尿病患者血清25羟维生素D3及血脂水平分析

摘    要:  目的  观察妊娠期糖尿病(GDM)患者体内血清25羟维生素D3[25(OH)D3]水平,探讨其对糖脂代谢水平的影响。  方法  选取2018年5月—2019年12月金华市人民医院产科收治的306例孕妇为研究对象, 检测其血清25(OH)D3水平及血糖、血脂相关指标, 根据口服葡萄糖耐量试验(OGTT)试验结果将孕妇分为妊娠期糖尿病组(74例)和正常妊娠组(232例), 比较2组孕妇血清25(OH)D3水平, 分析血清25(OH)D3水平与GDM的相关性,并比较妊娠期糖尿病患者不同亚组间的妊娠结局。  结果  妊娠期糖尿病组患者血清25(OH)D3水平为(55.20±0.06)nmol/L, 低于正常妊娠组的(64.92±1.10)nmol/L(P<0.05);妊娠期糖尿病组患者血糖及血脂相关指标水平均高于正常妊娠组,进一步分析相关性发现血清25(OH)D3水平与空腹血糖(FPG)、TC、TG呈负相关(均P<0.05);正常血清25(OH)D3水平的妊娠期糖尿病患者孕周为(39.20±0.16)周, 高于体内缺乏25(OH)D3的患者[(36.67±0.51)周], 且新生儿体重大于体内缺乏25(OH)D3的患者[(3.72±0.15)kg vs.(3.07±0.12)kg, P<0.05]。  结论  妊娠期糖尿病患者25(OH)D3水平低于正常妊娠孕妇, 且与FPG、TC、TG等糖脂代谢参数指标存在显著关联,若体内缺乏25(OH)D3可能会影响妊娠结局。

Analysis of serum 25 hydroxyvitamin D3 and blood lipid levels in patients with gestational diabetes mellitus

Abstract:  Objective  To view the 25-hydroxyvitamin D3[25(OH)D3] in patients with gestational diabetes mellitus (GDM), and view the influences of this level change on the pregnant woman blood sugar and lipid metabolism.  Methods  Total 306 pregnant women admitted to the Department of Obstetrics, Jinhua people's Hospital of Zhejiang Province from May 2018 to December 2019 were selected as the research objects, and their serum 25 (OH) D3 levels, blood glucose and lipid related indicators were detected. According to the results of OGTT test, pregnant women were divided into gestational diabetes mellitus group (n=74) and normal pregnancy group (n=232). The pregnancy outcomes of different subgroups of gestational diabetes mellitus were compared.  Results  The serum 25 (OH) D3 level in GDM group was (55.20±0.06) nmol/L, lower than that in the normal pregnancy group[(64.92±1.10) nmol/L, P < 0.05]. The levels of blood glucose and blood lipid in GDM group were higher than those in the normal pregnancy group. The serum 25 (OH) D3 level was negatively correlated with FPG, TC and TG (all P < 0.05). The gestational age of GDM patients with normal serum 25 (OH) D3 level was (39.20±0.16) weeks, which was higher than that in patients with 25 (OH) D3 deficiency[(36.67±0.51) weeks], and the neonatal weight was greater than that of the patients with 25 (OH) D3 deficiency[(3.72±0.15) kg vs. (3.07±0.12) kg, P < 0.05].  Conclusion  The level of 25 (OH) D3 in patients with gestational diabetes mellitus was lower than that in normal pregnant women, and it was significantly associated with FPG, TC, TG and other glucose and lipid metabolism parameters. If it was lack in the body, it would affect the pregnancy outcome.
