经阴道超声对围绝经期妇女阴道出血子宫内膜病变的诊断价值

目的探讨经阴道超声对围绝经期妇女不规则阴道出血子宫内膜病变的诊断价值。方法对126例围绝经期不规则阴道出血患者行经阴道超声（TVS）检查，观察其子宫内膜改变，并与相对应的子宫内膜病理结果进行对照。结果经阴道超声诊断围绝经期妇女不规则阴道出血子宫内膜病变的总符合率为94．44％，其中生理性内膜改变、良性内膜改变、内膜癌及非典型增生的诊断符合率分别为95．56％、94．67％及83．33％。以子宫内膜的正常厚度（10mm）为预测临界值，经阴道超声诊断围绝经期妇女不规则阴道出血子宫内膜病变的敏感性为87．65％，特异性为64．44％，阳性预测值为82．8％，阴性预测值为87．88％，假阳性为35．5％，假阴性为4．94％。结论经阴道超声对围绝经期妇女不规则阴道出血子宫内膜病变具有较高的诊断符合率及临床实用价值。

The diagnostic value of transvaginal ultrasonography (TVS) In endometrial lesions of perimenopausal women with irregular colporrhagia

Objective To investigate the diagnostic value of transvaginal ultrasonography （TVS） in endometrial lesions of perimenopausat women with irregular colporrhagia. Methods 126 cases of perimenopausal patients with irregular colporrhagia were examined by TVS and the endometrial lesions were observed. The pathologic results of endometrial lesions were compared with TVS. Results The total coincidence rate for endometrial lesions of perimenopausal women with irregular colporrhagia which were inspected by TVS was 94.44%. The diagnostic coincidence rate for physiologic endometrial changes,benign endometrial change, endometrial carcinoma, and atypical hyperplasia were 95.56%, 94.67%, and 83.33% respectively. The predicted critical value for normal uterine thickness is 10mm, the sensitivity for endometrial lesions of perimenopausal women with irregular colporrhagia examined by TVS was 87.65%, the specificity was 64.44%, the positive predictive value was 82. 8%, the negative predictive value was 87.88%, the false positive rate was 35.5%, and the false negative rate was 4.94%. Conclusion The transvaginal ultrasonography has high diagnostic coincidence rate and clinical practical value in endometrial lesions of perimenopausal women with irregular colporrhagia.
