子宫颈病变电环切术后的临床疗效观察

目的 探讨宫颈病变患者行子宫颈电环切术（loop e1ectrosurgical excision procedure，LEEP）后的临床疗效。方法 对宫颈薄层液基细胞学（liquid-based cytology test，LCT）、人孔头瘤病毒（human papillomavirus，HPV）检测阳性，阴道镜活检病理诊断为宫颈上皮内瘤样炳变（cervical intraepithelial neoplasia，CIN）Ⅰ58例、CINⅡ72例、CINⅢ95例（原位癌10例）患者行LEEP，切除组织标记后均行病理检查。结果 LEEP术后3个月随访CIN Ⅰ100％治愈，CIN Ⅱ98．6％治愈，CIN Ⅲ98．9％治愈，宫颈糜烂100％愈合，宫颈肥大89．8％完全恢复正常大小。术后6个月随访CIN 100％治愈。术后3、6、9、12个月随访患者HPV检测转阴率显著，10例原位癌术后随访1年正常。结论 LEEP术是目前临床上治疗宫颈病变的最有效方法。

Study on clinic curative effect of cervical intraepithelial neoplasia after LEEP conization

Objective To study on clinic curative effect and post-operative following-up of cervical intraepithelial neoplasia (CIN) after LEEP conization. Methods All 225 cases with LCT-positive and HPV-positive in our centre from 2003 to 2004 were biopsy by colposcopy, and were diagnosed as cervical intraepithelial neoplasia. The cases were divided into 3 groups according to lesion grade, which was CIN I (58), CIN II (72) and CIN III (95), and 225 patients were operated by LEEP. The samples of all LEEP operation were examined by pathology again. Results The curative rate of cervical erosion, cervical hypertrophy, CIN I, CIN II and CIN III were 100%, 89.8%, 100%, 98.6% and 98.9% respectively on 3 month after LEEP operation. HPV of 225 cases was negative on 3, 6, 9, 12 month after operation. Ten patients with CIN III were normal by LCT, HPV and colposcopy in one year. Conclusion LEEP operation is the most effective method for treatment of cervical lesion.
