16层螺旋CT门静脉成像技术显示门静脉及其侧支循环的价值

目的 探讨16层螺旋CT门静脉成像(CTP)显示门静脉及其侧支循环的临床价值.方法 回顾性分析对经临床证实并行16层螺旋CT门静脉成像的127例肝硬化门静脉高压患者的CT图像,进行多种不同方法的三维后处理并根据曲张静脉和侧支循环的部位、类型和程度进行分类.结果 127例患者的门静脉及其分支均显示良好,显示静脉曲张和侧支循环90例,其中胃周静脉曲张41例,食道胃底静脉曲张28例,脾肾静脉分流8例,门脐静脉分流10例,门脐腹股沟静脉分流3例.结论 CTP可无创性地显示门静脉及其侧支循环的部位、形态和程度,对临床诊断、选择和优化治疗方案、预后的评估有重要价值.

The value of 16-slice spiral CT portography in the display of portal vein and its collateral circulation

Objective To evaluate the clinical value of the technique of 16-slice CT portography in the display of portal vein and its collateral circulation. Methods We Retrospectively analyzed 16-slice spiral CT portography of 127 patients of hepatocirrhosis proved by clinic diagnosis, perform different 3 dimensional post-processing and classify by position, type and degree of varicosis and collateral circulation. Results Portal veins and its branches displayed very well of all 127 patients and CTP displayed varicosis and collateral circulation in 90 cases, thereinto perigastric varicosis 41 cases, esophageal and gastric fundic varicosis 28 cases, spleen and renal vein shunts 8 cases, portal and umbilical vein shunts 10 cases, portal umbilical and groin vein shunts 3 cases. Conclusion CTP is an noninvasive technique to display the position, type and degree of portal vein and its varicosis and collateral circulation. It is very important for clinical diagnosis, therapy protocol selection and optimization and evaluation of prognosis.
