纤维支气管镜引导下置入国产支架治疗严重气道狭窄15例分析

目的：探讨经纤维支气管镜(以下简称纤支镜)引导下安置国产支架治疗严重气道狭窄的应用价值。方法：15例严重气道狭窄患者均采用常规咽喉部局麻及环甲膜注射利多卡因后经纤支镜引导放置国产镍钛记忆合金支架(以下简称NT—支架)。结果：15例均一次性放置成功，呼吸困难症状迅速缓解。随访至发稿，12例恶性肿瘤患者，未做任何治疗的5例生存期4—6个月，坚持放射治疗及化学治疗的6例生存期12—18个月，2例良性气道狭窄放置后20个月仍存活，另有1例食管中上段癌放疗后致气管狭窄自1999年10月放置NT—支架至今37个月仍无支架移位，无任何呼吸困难症状，生活质量较好。结论：经纤支镜引导下放置国产NT—支架是治疗严重气道狭窄既经济又十分有效的方法。

Clinical Analysis of BF(bronchofiberscope)Guided Placement of Stents in the Treatment of Severe Tracheal Bronchial Stenosis: 15 Cases Attached

Objective:To evaluate the efficacy of BF guided placement of domestic made stents in the treatment of severe tracheal stenosis.Methods:Do mestic made NT-stents were placed in 15 cases of severe tracheal stenosis guided by BF.Results:The placement of NT-stent guided by BF was performed successfully in all cases. After furnished with stents, dyspnia was relieved immediately along with the pulmonary function improved. According to the results of follow-up, 5 malignant cases who had not any treatment else survived to 4-6 months. And 6 malignant cases taken with chemical therapy and radiotherapy survived to 12-18 months.The survival period of 2 benign cases was 20 months. Further more, one patient suffering from esophageal cancer with the most severe tracheal stenosis caused by radiotherapy has been alive for 37 months up to today without any dislocation of the stent or respiratory distress as well, having a better life quality. Conclusion: The method of BF guided to place domestic made stent would be a safe and economical effective operation on the severe tracheal bronchial stenosis cases.
