声门型喉癌切除术后喉功能重建

目的探讨声门型喉癌切除术后喉功能重建方法。方法对14例声门型喉癌进行了部分喉切除,分别用单蒂胸骨舌骨肌瓣(3例)、双蒂胸骨舌骨肌瓣(10例)、会厌瓣(1例)进行喉功能重建。结果全部病例术后均有不同程度的误咽,经适当训练后都很快恢复,全部拔除气管套管,拔管时间分别为术后12天～7个月,发音较正常人稍嘶哑,能正常交谈。随访满5年者3例,其中1例死于颈部转移,其余11例中还有1例术后4月死于局部复发和颈部转移,共12例无瘤生存至今。结论三种组织瓣取材方便,可以满足声门型喉癌切除术后不同缺损的修复需要,喉功能恢复满意。

Larynx function reconstruction after removal of glottic cancer

Objective To investigate the larynx function reconstruction after removal of glottic cancer . Methods In total 14 patients with glottic cancer treated with partial laryngectomy,their larynx function was reconstructed respectively using the single pedicle sternohyoid muscle flaps (3 cases),the double pedicle sternohyoid muscle flaps (10 cases) and the epiglottic flap (1 case).Results All patients had various degrees of aspiration and recovered by properl training.Patients were cannulated for 12 days to seven month after operation.All patients restored their normal speaking while their voice was hoarse.Three patients had been followed up for five years:one died of metastases in neck.One patient out of the other eleven cases died of recurrence and neck metastases four month after operation.In this series there were 12 patients who enjoyed tumor-free survival.Conclusion These three types of tissue flaps are easy to prepare,suitable for the reconstruction of larynx function,providing a satisfying clinical outcome.
