^99mTc—DTPA肾动态显像评价重度肾积水患肾残余功能的临床价值

摘    要:目的探讨核素^99mTc—DTPA肾动态显像评价重度肾积水患肾残余功能的临床价值。方法B超检查诊断为重度肾积水的患者29例(共30只患肾),应用核素^99mTc—DTPA行肾动态显像,同期行静脉肾盂造影(IVP),比较这两种方法评价患肾残余功能的差异。结果(1)在30只受检患肾中,有12只患肾IVP和^99mTc—DTPA肾动态显像均显示有残余功能,GFR为(22.32±3.55)ml/min;IVP不显影的患肾18只,其中8只^99mTc—DTPA肾动态显像提示患肾有残余功能,GFR为(13.36±4.54)ml/min,剩余10只患肾^99mTc—DTPA肾动态显像亦显示无残余功能。(2)^99mTc—DTPA肾动态显像结果显示有残余功能肾脏多于IVP(P〈0.05)。结论^99mTc—DTPA肾动态显像用于评价重度肾积水患肾残余功能较IVP更为灵敏,且能定量分析肾小球滤过功能。

The clinical value of 99m Tc - DTPA Renal dynamic imaging in evaluating residual function of the kidney with severe hydronephrosis

Abstract:Objective To evaluate the clinical value of the radionuclide 99mTc - DTPA renal dynamic imaging for measuring residual function of the kidney with severe hydronephrosis. Methods 29 patients( a total of 30 hydrops kidneys)who had severe hydronephrosis diag-nosed by B ultrasonography were compared with results obtained by 99mTc-DTPA renal dynamic imaging and intravenous pyelography respec-tively. Results (1) Among 29 patients with 30 hydrops kidney, 12 hydrops kidneys had shown residual function with the IVP and 99mTc-DTPA renal dynamic imaging, GFR was (22.32±3.55 )ml/min; IVP showed negtive in 18 hydrops kidney, but 99mTc -DTPA renal dynam-ic imaging showed positive in 8 cases, GFR was (13.36±4.54) ml/min,the otherl0 hydrops kidneys were showed no residual function by 99mTc-DTPA renal dynamic imaging as well; (2)99mTc-DTPA renal dynamic imaging had significant difference compare with IVP in resid-ual function test (P<0.05). Conclusion 99mTc-DTPA renal dynamic imaging is more sensitive than IVP in evaluating residual function of the kidney with severe hydronephrosis, and it also can measure GFR.
