剖宫产瘢痕妊娠的彩色多普勒超声分型及其在临床治疗中的应用

目的 探讨剖宫产瘢痕妊娠(CSP)的超声声像图分型在临床治疗中的应用价值.方法 选取2012年1月至2016年2月在解放军第105医院住院治疗的68例CSP患者,根据妊娠物部位、妊娠物与子宫浆膜层的距离不同而分为内生型和外生型,针对不同类型采取相应方法治疗,比较其治疗效果的差异.结果 68例CSP分为内生型55例,外生型13例,采用甲氨蝶呤(MTX)与子宫动脉栓塞(UAE)联合治疗,68例均治疗成功,其中66例均1次治疗成功,仅有2例进行了2次UAE联合MTX治疗,内生型的住院时间和出血量少于外生型(P＜0.05).结论 彩色多普勒超声不仅是早期诊断CSP的有效方法,而且对临床选择治疗方法具有重要指导作用.

Color doppler ultrasound classification and its application in clinical treatment of cesarean scar pregnancy

Objective To evaluate the effects of ultrasonic classification on the clinical treatment of cesarean scar pregnancy (CSP).Methods Sixty-eight patients with cesarean scar pregnancy (CSP) from Jan.2012 to Feb.2016 in the 105th hospital of PLA were enrolled.All patients with CSP were classified based on ultrasonic characteristics including pregnancy position,distance between pregnancy and uterine serosal layer.Individual treatment methods wereperformed according to the various ultrasonic characteristics,and the therapeutic effects were compared.Results Sixty-eight cases were divided into endogenous (n1 =55) and exogenous (n2 =13) type according to implantation site.Sixty-six cases were treated successfully with USE combined with MTX,and only two cases accepted twice USE and MTX treatment.The hospitalization time and blood loss of endogenous type wereevidently less than thoseof exogenous type.Conclusion Color Doppler ultrasound is an effective method for early diagnosis of CSP and plays an important role in clinical treatment.
