摘    要:目的探讨儿童乳磨牙牙体大面积缺损不同修复方式的临床效果。方法选择大面积缺损的300颗乳磨牙,随机分成三组,分别进行预成冠、铸瓷高嵌体、光固化复合树脂充填治疗。1年后复诊,参照美国公共健康协会的修正标准对修复效果进行综合评价。结果在牙体完整性和磨耗方面,三种修复方式组间差异无统计学意义;在修复体情况、继发龋、邻接关系方面,预成冠和铸瓷高嵌体优于树脂充填;而在边缘密合性方面,铸瓷高嵌体优于预成冠,预成冠又优于充填治疗。结论与树脂充填相比,虽然预成冠和铸瓷高嵌体的操作复杂、成本也较高,但是临床评价结果最好,在乳磨牙大面积缺损病例中可推广使用。

Abstract:Objective To Study of therapeutic effect of restorative methods for children’s badly broken-down deciduous molars.Methods 300 badly broken-down deciduous molar were selected and divided into prefabricated metal crown group,IPS E.mas onlay group,light cured composite resin filled group.All the patients were followed up for 1 year.The restorations quality was evaluated by a disciplined senior dentist using modified USPHS standard.Results 3 groups of restoration had no significant statistic difference in terms of tooth integrity and abrasion;IPS E.mas onlay group and prefabricated metal crown group were superior to resin filled group in respect to restoration situation,secondary caries and adjacent relations.In marginal adaption respect,onlay group preceded prefabricated metal crown group,and prefabricated metal crown group preceded resin filled group.Conclusion To compare with resin filled therapy method,although prefabricated metal crown and IPS E.mas onlay are put at a disadvantage for complex operation and high cost,their treatment effectiveness evaluation is better and worthy of clinical use.
