HDV感染者细胞免疫状态的研究

为探讨免疫反应在丁型肝炎发病中的作用，采用碱性磷酸酶抗碱性磷酸酶（ＡＰＡＡＰ）免疫酶染色技术，观察比较了３０例乙型肝炎病毒（ＨＢＶ）合并丁型肝炎病毒（ＨＤＶ）感染患者和３０例单纯ＨＢＶ感染者外周血及肝组织中的细胞免疫状态。结果发现ＨＢＶ合并ＨＤＶ感染者外周血ＣＤ８＋Ｔ细胞和ＣＤ１６＋细胞数显著高于单纯ＨＢＶ感染者，而与ＨＤＶ感染后是否抑制ＨＢＶ复制无关。在感染肝组织中，汇管区浸润细胞以ＣＤ４＋Ｔ细胞为主，小叶内坏死区中以ＣＤ８＋Ｔ细胞为主，与单纯ＨＢＶ感染者比较，两者差异无显著性。表明Ｔ细胞所介导的免疫应答在ＨＤＶ感染后所引起的肝细胞损伤中发挥着重要作用。

STUDIES ON CELLULAR IMMUNITY OF DELTA HEPATITIS

To exploit the role of immuopathegenesis in delta hepatitis the authors observed and compared the cellular immunity of 30 cases of patients with hepatitis B and 30 cases of patients with hepatitis B virus (HBV) coinfeted hepatitis D virus(HDV) by using APAAP immunochemistry staining. The results showed that numbers of CD8+ T and CD16+ NK /K lymphocytes were higher in the patients of coinfection than that of patients with pure HBV infection. In the infected hepatic tissue the infiltrated cells in portal area were mainly CD4+ T lymphocytes. By the contrast, in the intralobular and perilobular necrosis area CD8+ T lymphocytes were dominant. This type of infiltration was similar to that of HBV infection. It suggested that T cell mediated hepatic cell injury play an important role in HDV caused hepatitis.
