应用额部皮瓣修复面颊部软组织缺损

目的：报道额部皮瓣在面颊部缺损修复中的应用。方法：根据面颊部缺损情况，利用颞浅动脉额支所滋养的额部皮瓣转移及创缘组织瓣翻转形成衬里加额部皮瓣转移进行修复。结果：本组12例，缺损最大10cm×cm，最小3cm×3cm。额部皮瓣转移修复面颊部皮肤缺损8例；面颊部洞穿性缺损采用局部黏膜或皮肤衬里加额部皮瓣转移修复皮肤缺损4例。除2例额部皮瓣远端出现瘀斑、表皮部分液化脱落外，余皮瓣成活良好，色泽正常，存在感觉，面额部外观及功能恢复满意。结论：额部皮瓣血管走行恒定、表浅、血运丰富、血管蒂长、易于解剖和切取，是修复面颊部软组织缺损一种较好的方法。

Use of Forehead Flap for Repair of Buccal Soft Tissue Defects

Objective： To report the use of forehead flap for repair of buccal soft tissue defects. Methods： According to the condition of the defect, surgical repair was completed with local transfer of a forehead flap which is nourished by frontal branch of superficial temporal artery, an inner-lining turn-over flap at the wound margin tissues. Results： There were 12 cases in this cohort, with the largest defect being 10 cm × 8 cm and the smallest one being 3 cm × 3 cm in size. Surgical repair included 8 cases of buecal or facial skin defects using transferred forehead flap,4 cases of penetrating defects using local mucosa or skin as an inner lining plus transferred forehead flap. Except for 2 cases with ecchymosis formed to the distal end of flap, peeling or partial liquefaction of the epidermis, all cases showed surviving flaps with good color and sensation, satisfactory appearance and functional recovery of the fronto-facial region. Conclusion： Since forehead flaps contain blood vessels that are less variant, superficially located, with abundant blood supply, a long vascular pedicle, and easy to dissect and harvest, these may be favorable choice for repair of buccal soft tissue defects.
