48例经胆道镜液电碎石治疗肝内胆管残余结石的临床体会

目的：探讨经胆道镜液电碎石治疗肝内胆管残余结石的方法。方法：对48例经胆道镜液电碎石治疗肝内胆管残余结石病例的资料进行总结。其中左肝管结石20例，右肝管结石16例，左右肝管结石12例，单发结石13例，多发结石25例；经T管窦道入路45例，经胆肠吻合皮下预置空肠盲袢入路3例。结果：取石碎石成功率100％，合并胆道出血7例(14．6％)，无严重并发症。结论：经胆道镜液电碎石治疗肝内胆管残余结石是简便有效的方法，也是安全可靠的。

The Treatment of Retained Intrahepatic Biliary Calculi by the Electrohydraulic Lithotripsy in Cholangioscopy

Objective: To study the management of retained intrahepatic biliary calculi by the electrohydraulic lithotripsiy (EHL) in cholangioscopy. Methods: A study was performed of 48 patients, diagnosed with retained intrahepatic biliary caculi,and treated with EHL in cholangioscopy. Twenty patients were with left intrahepatic duct stone, 16 with right intrahepatic duct stone, 13 with single stone and 25 with multiple stone; The stones were reached via fistulae formed by T-tubes in 45 patiets and through the predisposed loop of jejunum under the subcutaneous of cholangio - jejunostomy. Results: all patients underwent the operation successfully, the effective rate was 100%. Seven patients presented with hematobilia, and none with severe complications. Conclusion: management of retained intrahepatic biliary caculi by EHL in cholangioscopy is easy, effective and safe.
