核素相位分析在心肌梗塞患者临床应用的初步探讨

摘    要:本文对38例心肌梗塞患者进行心脏血池显像相位分析和左心功能测定,以18例正常人作对照,结果显示:(1)相位分析法阳性者36例,而 LVEF 下降者30例,两者差异有显著性(P<0.05)。(2)前壁组,下壁组的相位角明显后滞,SD 增大;AVE、SD 测定结果与正常组对比,有非常显著性差异(P<0.01);LVEF、1/3EF、1/3FF 在前壁组、下壁组明显下降,与正常组对比有非常显著性差异(P<0.01),TPF 在前壁组、下壁组均明显延长,与正常组对比,有非常显著性差异(P<0.01)。提示:核素相位分析能客观地、直接地反映心肌梗塞患者的心室室壁的运动,显示其室壁运动的节段性改变;而 LVEF、1/3EF、1/3FF、TPF 可了解心室功能及代偿能力。两者结合起来,综合反映左心室的形态和功能的改变情况,在心肌梗塞的诊断及预后判断都有重要意义。

Abstract:The phase analysis of radionuclide ventriculograms and gated blood- pool images in 38 cases with myocardial infarction was conducted,and controlled with 18 normal persons.The result showed,(1)36 cases were positive by phase analysis,30 cases decreased in LVEF.The difference between the two groups was significant(P<0.05).(2)Phase angle were abnormal and SD were increase.LVEF,1/3EF,1/3FF were decreased. TPF were extension.Those result shows that whether groups of frant wall or low wall than normal persons were very significant(P<0.01). From above result,it demonstrated that phase analysis could objecti- vely and derectly shows motion of left ventric wall of myocardial infart- tion cases and section changes of left ventric of myocardial infarction cases. but LVEF,1/3EF,1/3FF and TPF may be studies left ventricular func- tion and compensatory ability;both the phase analysis and various para- meters of cardiac function has important role in estimation of prognosis.
