超声及X线在肾结核诊断中的价值比较及超声漏误诊原因分析

目的:进一步探讨超声在肾结核中的诊断价值,并与线对比分析;分析肾结核的超声漏误诊原因。X方法:回顾性分析了经手术病理证实的例肾结核个病肾的超声表现及其诊断情况,并与线检查(包括线肾造影及检查)结果进5862XXCT行对比分析。结果:肾结核声像图复杂,以肾积水及“结石”(部分为结核钙化灶)伴积水为主要表现,分别占、41.07%;肾结核的超声诊断正确率低于线综合诊断正确率,两者比较有显著性差异(21.43%XP<),但与平片加静脉肾盂造0.05影、穿刺或逆行造影、等线检查法单独比较时,超声诊断正确率与各种线检查法间无显著性差异(CTXXP值均大于)。0.05结论:声像图的多样性及对病史的忽视是超声误诊肾结核的主要原因,病变轻微是漏诊的主要原因。

Comparison of the diagnostic value of ultrasound and X-ray for renal tuberculosis and analysis of the causes of misdiagnosis of ultrasound

Objective: To assess the diagnotic value of ultrasound and X-ray for renal tuberculosis(RTB) and analyse the causes of misdiagnosis of ultrasound for RTB. Methods: We retrospectively studied the results of ultrasonography and X-ray of 58 patients with RTB (62 lesions)confirmed pathologically. Results: The ultrasonogram of RTB was complicated and nephrohydrosis (41.07%) and nephrohydrosis with nephrolithiasis (21.43%) were the major appearance.The correct diagnosis rate (CDR) of ultrasound was lower than that of X-ray (P<0.05). However, when compared it with that of APF plus IVP, PTP or RP, CT, respcectivly, there were no significant differences (P>0.05). Conclusion: The diversity of ultrasonogram and the neglect of the medical history are the major causes of misdiagnosis in ultrasound for RTB.
