摘    要:目的:探讨影响住院分娩新生儿Apgar评分的相关因素.方法:回顾性分析2007年1月~2008年12月在我院出生的新生儿的临床资料,收集Apgar评分及可能影响评分的产妇一般资料、产妇的临床特点及新生儿的临床特点等3方面因素,并采用有序Logistic回归进行分析.结果:孕妇年龄(P=0.014,OR=2.296,95%CI=1.964~3.157)、分娩方式(P=0.021,OR=2.757,95%CI=2.514~3.872)、妊高症产生情况(P=0.000 1,OR=3.377,95%CI=3.014~4.564)、分娩并发症情况(P=0.000 1,OR=3.475,95%CI=3.171~4.671)、新生儿体重(p=0.019,OR=2.130,95%CI=1.846~3.014)和新生儿出生畸形情况(P=0.0001,OR=3.885,95%CI=3.518~5.146)是影响新生儿1 minApgar评分的独立危险因素.结论:高龄产妇、产钳分娩、妊高症、分娩并发症、低体重儿、出生畸形是影响新生儿1 minApgar评分的独立危险因素,据此及早病因预防有望减少新生儿窒息的发生.

Study of the related factors affecting neonatal 1 min Apgar score
Abstract:Objective: To investigate the relevant factors impacting neonatal Apgar score. Methods: Clinical data of neonate in our hospital in January 2007~December 2008 were collected retrospectively, Three factors may affect the Apgar score were also collected, including maternal general information, the clinical features of maternal and neonatal clinical characteristics. Logistic regression were used in this analysis. Results: The maternal age(P=0.014,OR=2.296,95%CI =1.964~3.157),delivery mode(P=0.021,OR=2.757,95%CI=2.514~ 3.872), pregnancy hypertension (P=0.000 1 ,OR=3.377,95%CI=3.014~4.564) .delivery complications (P=0.000 1,OR=3.475, 95%CI= 3.171~4.671 ),birth weight(P=0.019, OR=2.130,95%CI =1.846-3.014) and the situation of birth deformities(P=0.0001 ,OR=3.885, 95%CI =3.518~5.146) are the independent risk factors that affecting neonatal 1 min Apgar score. Conclusion: Older mothers, forceps delivery, pregnancy hypertension, child birth complications, low birth weight, birth deformities are the independent risk factors which can impact neonatal 1 min Apgar score., etiology prevention is expected to reduce the occurrence of neonatal asphyxia.
