全腹腔镜与腹腔镜辅助直肠癌全直肠系膜切除术临床疗效观察

目的探讨全腹腔镜全直肠系膜切除术治疗直肠癌的临床疗效及优越性。方法将52例直肠癌患者随机分为全腹腔镜组(25例,观察组)和辅助腹腔镜组(27例,对照组),观察两组患者手术及术后并发症情况。结果两组患者手术时间、术中失血量、淋巴结清扫数目及切缘距病灶距离比较差异均无统计学意义(P0.05);观察组术后引流量、肛门排气时间及平均住院时间均低于对照组(P0.01);观察组近期术后并发症发生率低于对照组(P0.05)。结论两组手术方案具有同样的临床效果,全腹腔镜较腹腔镜辅助治疗直肠癌更具有微创性。

Clinical efficacy observation on total mesorectal excision for rectal cancer with total laparoscope and assisted laparoscope

Objective To explore the clinical efficacy and advantages of total mesorectal excision for rectal cancer with total laparoscope and assisted laparoscope.Methods Fifty-two patients with rectal cancer were randomly divided into total laparoscope group （25 patients） and assisted laparoscope group （27 patients）. The operation and postoperative complications of the patients in both groups were observed.Results There was no statistical significance between both groups in surgery time, intra-operative blood loss, numbers of cleaned lymph nodes and the distance between the incisal edge and the nidus（P 0.05）.The post-operative drainage volume,time of first flatus and average hospital stay of the patients in the total laparoscope group were lower than those of the assisted laparoscope group（P 0.01）. The incidence of recent post-operative complications of the patients in the total laparoscope group was lower than that of the assisted laparoscope group（P0.05）.Conclusion Two surgery plans have the same clinical efficacy. Treating rectal cancer with total laparoscope treatment has the advange of minimal wound.
