腹腔镜下超声刀筋膜内子宫切除术的应用

目的:探讨腹腔镜超声刀在筋膜内子宫切除术中的应用价值. 方法:将142例行子宫切除术的患者随机分成2组,研究组82例,采用腹腔镜下超声刀筋膜内子宫切除术(CISH),对照组60例采用传统腹式全子宫切除术(TH). 比较两组患者的手术时间、术中出血、术后恢复及随防情况. 结果:研究组手术成功81例,中转开腹1例. 两组时间比无明显差异,两组术中出血量、术后体温、术后恢复、术后平均住院日等有显著性差异(P<0.01).结论:CISH具有损伤小、出血少、恢复快、住院时间短等优点. 一次完成分离、止血、切割,避免了手术电灼伤,具有较好的临床应用前景.

Study of laparoscopic ultrasonic scalpel sheath hysterectomy

AIM: To investigate the value of ultrasonic scalpel sheath hysterectomy. METHODS: One hundred and forty-two patients with benign gynecological pathological changes who would have hysterectomy were divided into two groups: 82 patients in experimental group who would have classic intrafascial supracervical hysterectomy (CISH) and 60 patients in Traditional Gastrohysterectomy (TH) group. The two groups were compared in operation duration, amount of bleeding during operation, post-surgical recovery and follow-up results. RESULTS: Of the 82 cases in CISH group, 81 were successfully treated, and one patient underwent total abdominal hysterectomy (TAH) during the operation. There was no significant difference in operation duration between the two groups, but there were obvious differences in the amount of bleeding during operation, body temperature after operation, average postoperational hospital stay and recovery from the operation (P<0.01). CONCLUSION: The laparoscopic ultrasonic scalpel sheath hysterectomy has the advantages of minimal operating wound, low amount of bleeding, quick recovery, and short duration of hospitalization. Ultrasonic scalpel can achieve isolation , hemostasis and cutting in one step, and avoid electric burn. Ultrasonic scalpel is an ideal surgical option in treating benign uterine conditions.
