腰椎间盘突出合并椎管狭窄218例术式选择

目的：探析腰椎间盘突出合并椎管狭窄症手术术式选择。方法：分析２１８例腰椎间盘突出合并椎管狭窄病例临床表现，影像学资料，椎间盘突出侧别，椎管狭窄部位及与突出间盘间相互关系，对腰椎间盘突出症合并椎管狭窄的手术术式选择进行分析探讨。结果：２１８例中椎间盘旁侧型突出者１６９例，占７７．５％；中央型突出者４９例，占２２．５％。椎管狭窄中神经根通道狭窄者１０９例，占５０％；腰椎中央管狭窄者４１例，占１８．９％

Operative-style selection of 218 cases of lumber disc herniation with spinal canal stenosis

Aim: To analyze operative-styles of lumber discherniation with spinal canal stenosis. Methods: All information, including clinical menifestations, X-ray and CT scanfiles, side of herniation. position of stenosis and relation ofherniation with stenosis,were used to analyze the operativestyle selection. Results: The lumber disc herniations of lateral side were in 169 cases and of center were in 49 cases.The spinal canal stenosis of nerve root passage were in 109cases, of central canal were in 41 cases and mixed stenosiswere in 68 cases. Operative-style of windowing of laminawas performed in 84 cases. Hemilaminectomy was performedin 118 cases and total laminectomy in 16 cases. There were 7cases whose clinical syndromes were not relieved completely. Conclusion:It was established mainly for operative-styleselection by clinical menisfestation including careful deliberation of histories and physical findings. X-ray and CT scanof lumber spine could only promote assistances for operative-style selection. It was uncorrect that operative-style selection was depended on CT scan alonely. Must be avoidedthose tendencies: (1)Pursued smaller approach resulted inspinal stenosis to be remained or to be not removed thoroughly. (2) Destroyed more spinal structures resulted inspinal stability to be disordered.
