结直肠癌MDM2基因表达与生物学行为的关系

摘    要:目的 :探讨结直肠癌MDM2癌基因异常表达与肿瘤生物学行为间的关系 ,为判断预后提供客观依据 .方法 :应用免疫组化技术检测MDM2 ,p5 3和PCNA在 72例结直肠癌的表达特点 .结果 :伴有肝转移的结直肠癌组MDM2和p5 3蛋白表达阳性率 (5 9.3%和 78.0 % )与非肝转移组 (30 .0 %和5 5 .0 % )比较 ,差异均有显著性 (P <0 .0 5 ) .结直肠癌伴淋巴结转移组MDM2和 p5 3蛋白表达阳性率 (5 8.8%和 79.4 % )明显高于与无淋巴结转移组 (2 8.9%和 5 2 .6 % ,) ,两者间有显著的统计学意义 (P <0 .0 5 ) ;有静脉侵袭组MDM2表达阳性率 (6 0 .7% )高于无静脉侵袭组 (31 .8% ) ,MDM2基因异常表达同静脉侵袭有关 (P <0 .0 5 ) .结论 :MDM 2和p5 3基因蛋白阳性率与淋巴结转移、肝转移有明显相关性 ,MDM2和p5 3联合检测对评估结直肠癌预后有重要的临床价值

Immunohistochemical detection of MDM2 protein in colorectal cancer: Relevance to tumor pathological features
Abstract:AIM: This study was undertaken to examine the presence of immunoreactivity against MDM2 protein in colorectal cancer and its relevance to the biological behaviors of tumors. METHODS: Expressions of MDM2 and p53 as well as PCNA labeling index ( PCNA LI ) were examined in 72 cases of colorectal cancer by immunohistochemical methods. RESULTS: The positive rates of immunostaining against MDM2 and p53 protein in the group of liver metastasis were 59.3% and 78.0%, respectively. The group with liver metastasis differs significantly from the group without metastasis ( P <0.05). Moreover, the detection rates of immunostaining against MDM2 and p53 protein between the group with lymph node metastasis (58.8%, 79.4%) and the group without metastasis (28.9%, 52.6% ) were statistically significant ( P <0.05, P <0.05). Expression of MDM2 between the groups with venous invasion (60.7%) and with noinvasion (31.8%) was also remarkably different ( P <0.05). PCNA LI was not significantly associated with various parameters like sex, sites of tumor occurrence, tumor size, pathological grading, depth of invasion, liver metastasis, lymph node metastasis and venous invasion in colorectal cancer. CONCLUSION: This study suggests that the detection of MDM2 and p53 immunoreactivity seems to be of greater value as a biomarker in metastasis of colorectal cancer into the liver, lymph node and venous system.
