热休克蛋白70在外阴白斑和外阴鳞癌中的表达及临床意义

摘    要:目的:研究热休克蛋白70(HSP70)在外阴白斑和外阴鳞癌中的表达,探讨其在外阴白斑和外阴鳞癌发生、发展中的意义.方法:应用免疫组化法检测外阴白斑80例,不典型增生13例,外阴鳞癌48例和正常皮肤组织20例中HSP70的表达.结果:HSP70在外阴白斑各分型中阳性率分别为:增生型31.2%(10/32),硬化萎缩型9.1%(2/24),混合型29.2%(7/24),不典型增生型46.2%(6/13)2在正常皮肤组织中阳性率为0(0/20);在外阴鳞癌中阳性率为62.5%(30/48),在鳞癌、外阴不典型增生、外阴白斑中HSP70的表达和正常组织表达率相比有非常显差异(P<0.01).HSP70在I级,Ⅱ级,Ⅲ级外阴鳞癌中阳性表达率分别为55.0%(11/20),55.6%(10/18),90.0%(9/10).其表达在鳞癌各组织分级之间比较无明显差异(P>0.05).结论:HSP70在外阴白斑、外阴不典型与鳞癌中的表达情况存在明显差别.HSP70的异常表达可能与外阴鳞癌的发生与发展有关。

Abstract:AIM: To investigate the expression of HSP70 and to evaluate its clinical significance in the occurrence and development of genitalia leukoplakia and squamous cell carcinoma of vulva (SCC). METHODS: Using the method of immunohistochemical, human HSP70 was detected in 80 cases of genitalia leukoplakia, 13 cases of squamous cell carcinoma of vulva and 20 cases of normal skin. RESULTS : The positive rates of HSP70 in different types of genitalia were 31.2% in hyperplasia type, 9.1% in sclerotic type, 29.2% in mixed type, 46.2% in atypical hyperplasia and 0% in normal skin, respectively. The positive rate of HSP70 was 62.5% in patients with SCC, which was significantly higher than that of genitalia leukoplakia type and normal skin ( P <0.01). The positive rates of HSP70 in the different grades of SCC were 55.0% (11/20) in grade I, 55.6% (10/18) in grade II and 90.0% (9/10) in grade III ( P >0.05). CONCLUSION: The results suggest that HSP70 expression is different in different tissues and the aberrant expression of HSP70 is closely related to the tumorgenesis and progression of SCC.
