联合手术治疗先天性髌骨脱位的体会

目的 探讨联合手术治疗儿童先天性髌骨脱位的疗效。方法 先天性髌骨脱位患儿5例，采用联合手术的方法对髌骨脱位进行治疗；手术方法包括膝外侧软组织广泛松解，膝内侧软组织紧缩，股内侧肌移位和髌韧带止点移位或半侧髌腱内移术。随访4个月～4年。结果 患者均获得满意的功能和稳定的膝关节，运动能力明显提高；所有患者无术后发生再脱位和膝关节活动受限。结论 采用联合性手术的方法治疗儿童先天性髌骨脱位，可取得明显疗效。

Operative treatment of congenital disclocation of patella through combination operation

Objective To study the curative effectiveness of combination operation in the treatment of congential dislocation of patella. Methods From Jan. 2000 to March 2004, 5 cases of congenital dislocation patella were treated through combination operation and these cases were reviewed. There were 2 cases of mono-knee dislocation and 3 cases of bi-knee dislocation. The combined operations included lateral retinacular releas, medial retinacular tightening of the knee, vastus medialis transfer to patella, medial and distal transfer of the half patellar tendon, medial transfer of the tibia tubercle. They were followed up on average for 4 months to 4 years. Results All patients had the stable knee joint and a full range of motion, and motor functions were obviously improved after operation. The re-dislocation and restriction of the knee joint motion were not found. Conclusion The combined operation for the congenital dislocation of the patella in children can obtain satisfactory curative effectiveness.
