BiPAP模式无创通气对急性左心功能衰竭的呼吸支持作用研究

摘    要:目的:观察经面罩双水平气道正压通气(BiPAP)对急性左心功能衰竭患者的呼吸支持作用及临床效果,为急性左心功能衰竭患者呼吸支持方式提供参考。方法:选择102例急性左心功能衰竭患者,随机分为研究组及对照组,所有患者均给予强心、利尿、扩血管及镇静等抗心衰治疗,对照组患者给予常规鼻导管吸氧,研究组患者经面罩给予BiPAP无创通气支持。呼吸支持8小时候对比两组患者临床治疗效果,血流动力学参数及血气分析变化。结果:研究组临床有效率高于对照组,研究组HR低于对照组,SBP高于对照组,HCO2-低于对照组,PaCO2低于对照组,PaO2高于对照组,BE低于对照组,差异具有统计学意义。结论:BiPAP模式无创通气模式呼吸支持能够改善患者呼吸功能、纠正酸碱失衡,发挥对急性左心功能衰竭的治疗作用。

Study to Atraumatic Ventilatory Support with BiPAP Pattern on Acute Left Heart Failure

Abstract:Objective:To observe clinical efficacy and respiration support effect of ventilatory support with Bi-level positive airway pressure(BiPAP)pattern on acute left heart failure.Methods:102 patients with acute left heart failure were selected and randomly divided into control groups and research groups,all patients were treparyed with cardiotonic,diuretic,expanding vascellum medicine and tranquillizer for rivalry heart failure.the paitients in control group breathe oxygen by nasal catheter and the paitients in research groups received ventilatory support with BiPAP pattern,After 8h,clinical efficacy were contrasted,parameter of hemodynamics and result of blood gas analysis were compared between control group and reserch group.Results the clinical effective power was higher in reserch group than in control,heart rate(HR) was lower and systolic blood pressure(SBP) were higher in reserch group man in control,HCO2-,PaO2and BE was lower in reserch group than in control,and PaO2 was higher in reserch group than in control.Conclusion:Ventilatory support with BiPAP pattern could improve respiratory function,retrieve acid-base imbalance and have a therapeutic action in acute left heart failure.
