摘    要:目的:颈动脉支架置入术(CS)是目前治疗颈动脉重度狭窄预防缺血性脑中风的介入领域热点之一。如何防止该操作过程中,微栓子脱落入脑可能发生脑功能不全是该治疗方法成功的关键。本文在CS操作中常规采用Filter Wire脑保护装置用于上述目的作一些探讨。方法:将36例德国患者在FilterWire脑保护装置下完成颈动脉支架置入术,对术中及术后近期并发症作一总结。结果:围手术期,患者死亡率及中风率是0%,术后发生一过性脑缺血的患者人数是1例(2.77%)。该患者术前有长期低血压病史,行药物治疗,症状消失。结论:常规用FilterWire脑保护装置完成颈动脉支架置入术,其近期并发症发生率很低,而且该装置易于操作,对于国内全面开展该治疗项目有一定的借鉴。但是,其长期随访效果尚无定论。

Abstract:Objective: The role of carotid stenting has been limited by its potential for producing embolic debris. To counteract such problems, we evaluated the efficacy of a FilterWire cerebral protection device in the prevention of embolic events during carotid stenting. Methods: Thirty six patients with severe carotid atheromatous stenoses were treated by stent implantation under FilterWire cerebral protection in department of interventional radiology, Dortmund hospital, Germany. Pretreatment antiplateleted therapy was administered . The short - term outcomes are recorded. Results: The technical success rate was 100% . The 30 - day mortality/ morbidity rates as follows' death, 0% ; major strokes, 0% ; TIAS, 2.77 % (1 case), the patient who suffered from minor stroke had a long - time hypotension, and was complete recovery with medicine therapy. Conclusion: Cerebral protection with the FilterWire device is easy safe and effective in protecting the brain from cerebral embolism. Experience from a single group of operators demonstrates carotid stenting under the FilterWire device can be performed with an acceptable 30 - day complications rate. But late follow - up and randomized carotid endarterectomy versus carotid stenting with cerebral protection devices are extremely necessary.
