肾动态显像加肾小球滤过率测定与CT、静脉肾盂造影联合临床应用

摘    要:目的探讨三种方法在泌尿外科疾病中的联合应用价值。方法 (1)肾动态显像加肾小球滤过率(GFR)测定:设备为Philips Fort SPECT,99Mo-99Tcm发生器由北京原子能研究所提供,配套药盒DTPA(注射用亚锡喷替酸)由江苏省原子医学研究所江原制药厂提供。(2)Philips16排螺旋CT,层厚5mm,螺距5mm。(3)Philips Essenta RC数字胃肠机,造影剂碘佛醇注射液(50ml:33.9g)。(4)分组:①肾动态显像加GFR测定与CT联合检查组。②肾动态显像加GFR测定与静脉肾盂造影联合检查组。③静脉肾盂造影肾、输尿管未见显示(单侧或双侧)组。结果肾动态显像加GFR测定与CT、静脉肾盂造影符合率高。CT、静脉肾盂造影为临床提供详细解剖信息,肾动态显像加GFR测定提供详细分肾功能量化信息,功能评价优于肾盂静脉造影。结论肾动态显像加GFR、CT组合为评价肾脏、输尿管的解剖和功能最佳选择。

Abstract:Objective To discuss the value of conjoint application of the three methods.Methods (1)Renal dynamic imaging plus GFR assay: the facilities were Philips Fort SPECT and 99Mo-99Tcm generator provided by Beijing Atomic Energy Research Institute,and the matching reagent cases DTPA(pentetate acid and stannous chloride for injection)were supplied by Jiangyuan pharmaceutical factory of Jiangsu Province Atomic Medicine Research Institute.(2)The CT was Philips 16-row spirals,with each layer 5 mm thick and each screw distance 5 mm.(3)Digital gastrointestinal X-ray machine was Philips Essenta RC,and the contrast medium was ioversol injection(50 ml:33.9 g).(4)The groups were divided as renal dynamic imaging plus GFR assay combining with CT scan,renal dynamic imaging plus GFR assay combining with intravenous pyelography,and the group of mono or both sides kidney and ureter no display in intravenous pyelography.Results Renal dynamic imaging plus GFR assay and CT plus intravenous pyelography obtained high congruous rate.CT plus intravenous pyelography offered particular anatomic informations for clinic,while renal dynamic imaging plus GFR assay provided particular renal functional quantitative informations of each kidney, thus the function evaluations excelled pure intravenous pyelography.Conclusions The combination of renal dynamic imaging plus GFR assay and CT scan was the best of all choices in evaluating kidney and ureter anatomy and function.
