p16蛋白和CD44V6在胃癌中的表达及临床意义的研究

目的　探讨 p16蛋白 ,CD44V6在胃癌的发生发展及转移中的临床意义。 方法　应用免疫组织化学方法对 46例胃癌及癌旁组织中 p16和CD44V6表达进行检测。 结果　胃癌组织中 p16蛋白阳性率 (8/ 46 ,2 1.0 5 % )明显低于癌旁组织中表达率 (2 7/ 46 ,5 8.70 % )。而CD44V6在胃癌中的表达率为 (71.74% ,33/ 46 ) ,癌旁组织中未见CD44V6表达。两者的表达与年龄、性别、肿癌大小无相关性 ,而与有无淋巴结转移显著相关。p16与分化程度有显著差异 ,而CD44 6则与之无关。在TNM分期中 ,CD44V6在Ⅲ、Ⅳ期中的表达显著高于Ⅰ期中 ,而p16在Ⅰ期中的表达率明显高于Ⅱ、Ⅲ、Ⅳ期中的表达率。p16与CD44V6两者在胃癌中的表达无相关性。但有淋巴结转移组(2 7例 )中 ,p16阳性表达率 3 .70 %明显低于CD44V6阳性表达率 88.89% ,且有显著差别 (P <0 .0 5 )。 结论　p16蛋白、CD44V6是胃癌发生发展的重要因素 ,特别是CD44V6可作为胃癌淋巴结转移的重要指标 ,p16蛋白、CD44V6可为临床诊断胃癌、判断预后、指导手术提供重要依据

The Expression of p16 Protein and CD44V6 in Gastric Cancer and Their Significance

Objective To study the clinical significance and effect of the expression of p16 protein and CD44V6 in the occurrence and development of gastric carcinomas.Methods p16 protein and CD44V6 expression were detected in 46 specimens of gastric carcinomas and pericarcinoma tissues by the methods of immunohistochemistry.Results The positive rate of p16 protein expression 8/46( 21.05 %) was significantly lower than that in pericarcinoma tissues 27/46(58.70%).The positive rate of CD44V6 was 33/46(71.74%) in gastric carcinomas,but it was not expressed in pericarcinoma tissues.The positive expression of p16 protein and CD44V6 were correlated with lymph node metastasis of gastric carcinomas.The positive expression of p16 protein were correlated with degree of differentiation and clinical stage.CD44V6 expression was correlated with clinical stage,but not with degree of differentiation.The expression of p16 protein and CD44V6 were not with correlated with sex,age and tumor size.But in the gastric carcinoma with lymph node metastasis,the positive rate of p16 protein (3.70%) was significantly lower than that of CD44V6 (88.89%) ( P <0.05).Conclusion The abnormal expressions of p16 protein and CD44V6 were important factors in the occurrence,development of gastric carcinoma,specially CD44V6 was an important index in the lymph node metastasis of gastric carcinoma.So the detection of p16 protein and CD44V6 imply important significance for gastric carcinoma dignosis,pronosis and treatment.
