带锁髓内钉治疗胫骨骨折72例临床分析

目的：探讨扩髓带锁髓内钉治疗胫骨不稳定性骨折的治疗效果。方法：对72例胫骨不稳定性骨折患者行扩髓带锁髓内钉治疗,闭合性骨折50例,开放性骨折22例,全部应用静力性固定。结果：平均随访时间12个月（6-20个月）,采用Joh-ner-Wruh评分标准,优30例,良37例,一般5例。骨折平均愈合时间：闭合性骨折15周（10-22周）,开放性骨折19周（15-36周）,4例延迟愈合,无深部感染、骨髓炎、畸形愈合及骨不连。结论：扩髓带锁髓内钉是治疗胫骨不稳定性骨折较理想的方法,可促进骨折愈合,并发症较少。

Treatment of tibial fractures with interlocking intramedullary nail in 72 cases

Objective To discuss the effect of reamed interlocking intramedullary nailing in the treatment of unstable tibial fracture.Method Total 72 cases with unstable tibial fracture(50 of closed fracture and 22 of open fracture) were treated with reamed interlocking intramedullary nailing.Results The mean follow-up time was 12 months(6~20 months).The results were judged by Johner-Wruh scoring criterion:30 cases had excellent effect,37 had good effect and 5 had effect.The average healing time was 15 weeks(10~22 weeks) in closed fracture and 19 weeks(15~36 weeks) in open fracture.Four cases got delayed union,no deep infection,osteomyelitis,malunion and nonunion occurred.Conclusion Reamed interlocking intramedullary nailing is an ideal method for the treatment of unstable tibial fracture,and it can promote fracture healing and with less complications.
