摘    要:目的:探讨钼靶X线摄影与MRI联合对隐匿性乳腺癌的诊断价值。方法:回顾分析因腋窝肿块就诊,临床乳房内未触及肿块,后经腋窝肿块切除活检证实为淋巴结转移性腺癌的患者共33例,术前均行钼靶X线摄影与动态增强MRI检查。结果:MRI诊断正确30例,诊断准确率为90.91%,显示钙化4例,钙化显示率为12.12%;钼靶X线摄影诊断正确24例,诊断准确率为72.73%,显示钙化22例,钙化显示率为66.67%。两者差异均有统计学意义(P<0.01)。结论:动态增强MRI对隐匿性乳腺癌的诊断价值明显优于X线钼靶,钼靶对钙化较敏感,两者联合对早期发现、早期诊断、早期治疗隐匿性乳腺癌具有非常重要的临床意义。

Abstract:Objective To study the value of occult breast cancer diagnosed by dynamic contrast enhanced-MRI and digital mammography.Method Clinical data from April 2005 to April 2011 were retrospectively analyzed,and 33 cases with no palpable tumor in breast visited because of the tumor in armpit,then proved to be metastasis adenocarcinoma of lymph node after the examination of the tumors cut in armpit.All cases accepted dynamic contrast enhanced-MRI and digital mammography.Results The diagnostic accuracy of occult breast cancer with MRI and digital mammography were 90.91%(30/33) and 72.73%(24/33)(P<0.01),respectively.X-ray showed calcifications(n=22) while MRI showed calcifications(n=4).Conclusion Dynamic contrast enhanced-MRI is superior to digital mammography in the diagnosis of occult breast cancer,but inferior in the sensitivity of calcification.So dynamic contrast enhanced-MRI united with digital mammography will play a significant role in the diagnosis of occult breast cancer.
