Tei指数评价正常胎儿和子痫前期胎儿心功能的应用价值

目的：探讨Tei指数在评价正常胎儿和子痈前期胎儿心脏功能中的应用价值。方法：测量300例正常胎儿（22～27周^＋6、28—36周^＋6、37—42周各100例）左、右心室Tei指数，分析孕周、胎心率对Tei指数的影响，测量重度子痫前期组46例，轻度子痫前期组68例和与其孕龄匹配的70例正常胎儿左、右心室Tei指数，分析三组间比较差异有无统计学意义。结果：300例正常胎儿各孕周组胎儿右心室Tei指数均高于左心室，但差异无统计学意义（P〉0．05）；左、右心室Tei指数与孕周呈负相关（左心室r=-0．77，P〈O．01，右心室r=-0．72，P〈0．01），与胎心率无相关性。重度子痫前期组与轻度子痫前期组胎儿左、右心室Tei指数均显著高于正常胎儿组，差异有统计学意义，P〈0．05。结论：Tei指数是一种简单可靠的定量综合评价子痫前期胎儿心肌受累的指标。

Evaluation of Tei index in normal fetuses and fetuses with mild and severe pre-eclampsia for fetal cardiac function

Objective To analyse the value of Tei index for assessing fetal cardiac function in normal fetuses and fetuses with Pre-eclampsia. Method Fetal echocardiograms were performed on 300 normal fetuses who were divided into three groups(22～27~(+6),28～36~(+6), 37～42 gestational weeks).The correlation between Tei index and gestational weeks,fetal heart rate were analyzed.Left ventricular(LV) and right ventricular(RV) Tei index were measured in 46 cases with severe pre-eclampsia,68 mild preeclampsia and 70 normal fetuses, and the difference was compared between the three groups.Results There was not significant difference of Tei index of LV and RV with three gestational weeks in 300 normal fetuses(P>0.05);Tei index of LV and RV decreased linearly with advancing gestational in normal group(LV:r=-0.77,P<0.01;RV:r =-0.72,P<0.01),and independent on heart rate.Tei index in mild and severe pre-eclampsia groups were significantly higher than that of the control group(P<0.05).Conclusion Tei index is a simple and reliable Doppler index of quantitative assessing overall ventricular function in fetuses with pre-eclampsia.
