血小板参数及糖化血红蛋白的联合检测在糖尿病患者微血管病变临床意义的探讨

目的:探讨血小板参数(PLT、MPV、PDW)及糖化血红蛋白(HbA1c)的联合检测在糖尿病患者微血管病变的临床意义。方法:采用全自动血球计数仪Sysmex xs-800i对62例糖尿病患者及38例体检健康者进行PLT、MPV、PDW检测,同时在全自动生化分仪BECKMANLX-20上采用免疫比浊法进行HbA1c检测。结果:发现糖尿病组与正常对照组的检测结果比较,差异有统计学意义,PLT为(154.1±51.2)×109/L(P<0.05),MPV为(12.71±2.32)fL(P<0.01),PDW为(16.71±1.39)L(P<0.01),HbA1c为(9.29±2.04)%(P<0.01)。结论:检测结果表明血小板参数的不同变化与糖尿病患者微血管病变有关,而HbA1c变化与其病损程度和预后相关,两者具有互补作用,既可早期发现微血管病变的存在,又可提示病损程度,在监测糖尿病患者微血管病变的发生及预后判断等方面具有重要的临床意义。

Clinical significance discussion of the joint detection of platelet parameters and glycosylated hemoglobin on diabetic microangiopathy

Objective To discuss the clinical significance of the joint detection of platelet parameters(PLT,MPV,and PDW) and glycosylated hemoglobin(HbAlc)on diabetic microangiopathy.Methods Detection of PLT,MPV and PDW was made in 62 diabetic patients and 38 healthy people by automatic blood-cell counter Sysmex xs-800i while HbAlc detection was made in an immune turbidimetry method by automatic biochemical analyzer BECKMAN LX-20.Results There was significant difference on the detection results between the diabetic group and the normal control group:PLT(154.1±51.2)×109/L(P<0.05),MPV(12.71±2.32)fL(P<0.01),PDW(16.71±1.39)L(P<0.01),HbA1c(9.29±2.04)%(P<0.01).Conclusion Detection results show that different changes of platelet parameters are relevant to diabetic microangiopathy and that HbAlc change is relevant to lesion degree and prognosis.Both of the changes complement each other.They not only early detect the existence of microangiopathy,but also indicate lesion degree,which perform important clinical significance in the monitoring of diabetic microangiopathy occurrence and prognosis,etc.
